Description
Overview of the medicine
Enliven 100 mg Capsule contains Pregabalin, which is an anti-epileptic and neuropathic pain-relieving medication. It is used to treat neuropathic pain, fibromyalgia, and as an adjunctive therapy for partial onset seizures in adults.
Uses & Indications
Dosage
Adults
Initial dose 75 mg twice daily or 50 mg three times daily. May be increased to 150 mg twice daily or 100 mg three times daily after 3-7 days based on response and tolerability. Maximum dose 300 mg twice daily or 200 mg three times daily (total 600 mg/day).
Elderly
Reduced dosage may be required due to potential age-related decline in renal function. Start with a lower dose and titrate cautiously.
Renal_impairment
Dosage adjustment is necessary. For CrCl 30-60 mL/min, start with 25-50 mg twice daily; for CrCl 15-30 mL/min, start with 25-50 mg once daily; for CrCl <15 mL/min, 25 mg once daily or 50 mg every other day. Hemodialysis patients should receive a supplemental dose after dialysis.
How to Take
Enliven capsules should be swallowed whole with water, with or without food. Do not chew, crush, or open the capsule.
Mechanism of Action
Pregabalin binds with high affinity to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding modulates the release of several neurotransmitters, including glutamate, norepinephrine, and substance P, which are involved in pain transmission and seizure activity. It does not bind directly to GABA A or B receptors.
Pharmacokinetics
Onset
Pain relief can be experienced within a week, seizure control varies.
Excretion
Primarily excreted unchanged in urine (approximately 98%) via renal excretion.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1.5 hours. Bioavailability >90% and is not affected by food.
Metabolism
Negligibly metabolized in humans; about 0.08% of dose found as N-methylated derivative.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any component of the formulation.
- Angioedema history.
Drug Interactions
Thiazolidinediones (e.g., rosiglitazone, pioglitazone)
May cause weight gain and peripheral edema when co-administered. Monitor for fluid retention and adjust dose if necessary.
CNS Depressants (e.g., opioids, benzodiazepines, alcohol)
May enhance CNS depressant effects, including respiratory depression and sedation. Concomitant use should be approached with caution.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Management is supportive, including gastric lavage if indicated, and monitoring of vital signs. Hemodialysis can remove pregabalin from the circulation.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted in breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
