Description
Overview of the medicine
Enviral (Entecavir) is an oral antiviral drug used to treat chronic hepatitis B virus (HBV) infection in adults and children aged 2 years and older with evidence of active viral replication and persistent elevations in serum aminotransferases or histologically active disease.
Uses & Indications
Dosage
Adults
Nucleoside-naïve adults: 0.5 mg orally once daily. Lamivudine-refractory adults: 1 mg orally once daily. Take on an empty stomach (at least 2 hours after a meal and 2 hours before the next meal).
Elderly
No specific dosage adjustment is required based on age, but renal function should be monitored due to potential age-related decline in renal function.
Renal_impairment
Dosage adjustment is recommended for patients with creatinine clearance <50 mL/min (including hemodialysis or CAPD). Consult prescribing information for specific dose adjustments.
How to Take
Take Enviral orally, once daily, preferably on an empty stomach (at least 2 hours after a meal and 2 hours before the next meal). This is to ensure optimal absorption.
Mechanism of Action
Entecavir is a guanosine nucleoside analog with activity against HBV polymerase. It is phosphorylated to its active triphosphate form, which competes with the natural substrate, deoxyguanosine triphosphate, to inhibit all three functions of the HBV polymerase: priming of the HBV DNA polymerase, reverse transcription of the negative strand from the pregenomic RNA, and synthesis of the positive strand of HBV DNA.
Pharmacokinetics
Onset
Significant reduction in HBV DNA levels can be observed within weeks of initiating therapy.
Excretion
Primarily excreted unchanged in the urine, with approximately 62-73% of a single dose recovered unchanged in the urine.
Half life
The intracellular half-life of the active triphosphate is 128-149 hours. The plasma elimination half-life is approximately 21-24 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring between 0.5 to 1.5 hours. Bioavailability is approximately 100% when taken without food.
Metabolism
Entecavir is not a substrate, inhibitor, or inducer of the cytochrome P450 (CYP450) enzyme system.
Side Effects
Contraindications
- •Hypersensitivity to entecavir or any component of the formulation.
- •Patients with known severe renal impairment without dose adjustment.
Drug Interactions
Drugs eliminated by renal excretion
Co-administration of entecavir with drugs that reduce renal function or compete for active tubular secretion may increase serum concentrations of either entecavir or the co-administered drug. Examples include tenofovir disoproxil fumarate, lamivudine, adefovir dipivoxil.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Limited experience with entecavir overdose. In case of overdose, patients should be monitored for evidence of toxicity, and standard supportive treatment should be applied. Hemodialysis can remove approximately 13% of the entecavir dose over 4 hours.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether entecavir is excreted in human milk; due to the potential for adverse effects in nursing infants, women should be advised not to breastfeed if taking entecavir.
