Description
Overview of the medicine
Envitor is an antiviral medication containing Entecavir, primarily used to treat chronic hepatitis B virus (HBV) infection in adults and children aged 2 years and older. It works by interfering with the replication of the hepatitis B virus.
Uses & Indications
Dosage
Adults
Adults and adolescents (≥16 years of age): 0.5 mg once daily. For lamivudine-refractory patients or those with known lamivudine resistance, 1 mg once daily.
Elderly
No specific dosage adjustment is required based on age, but renal function should be monitored due to potential age-related decline in kidney function.
Renal_impairment
Dose adjustment required based on creatinine clearance (CrCl): CrCl 30-50 mL/min: 0.5 mg every 2 days; CrCl 10-29 mL/min: 0.5 mg every 3 days; CrCl <10 mL/min or hemodialysis/CAPD: 0.5 mg every 5-7 days.
How to Take
Take orally on an empty stomach (at least 2 hours after a meal and 2 hours before the next meal). This improves absorption.
Mechanism of Action
Entecavir is a guanosine analogue nucleoside reverse transcriptase inhibitor (NRTI). It is phosphorylated intracellularly to the active triphosphate form, which competes with natural deoxyguanosine triphosphate and effectively inhibits all three functions of the HBV polymerase: priming of the HBV polymerase, reverse transcription of the negative strand from the pregenomic messenger RNA, and synthesis of the positive strand HBV DNA.

Viral load reduction typically starts within weeks of treatment.
Excretion
Primarily excreted unchanged by the kidneys, with approximately 62-73% of the dose recovered in urine.
Half life
Terminal elimination half-life is approximately 128-149 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur between 0.5-1.5 hours. Bioavailability is approximately 100%.
Metabolism
Not a substrate, inhibitor, or inducer of cytochrome P450 enzymes. Not significantly metabolized by the liver.
Side Effects
Contraindications
- Hypersensitivity to Entecavir or any component of the formulation.
- Lactate acidosis history (use with caution).
Drug Interactions

Drugs that reduce renal function (e.g., cyclosporine, aminoglycosides): May increase Entecavir concentrations. Monitor renal function and Entecavir levels.

Nephrotoxic drugs (e.g., NSAIDs, tenofovir): Concomitant use with drugs eliminated by renal tubular secretion may increase serum concentrations of either Entecavir or the coadministered drug.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. In clinical trials, subjects who received up to 20 mg/day for up to 28 days and single doses up to 40 mg experienced no unexpected adverse events. Overdose management should include general supportive measures.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Entecavir is excreted in human milk; caution should be exercised when administered to a nursing woman.
