Description
Overview of the medicine
Ephidin, derived from Ephedrine, is a sympathomimetic drug used as a bronchodilator, decongestant, and central nervous system stimulant. It is effective in treating conditions like bronchial asthma, narcolepsy, and hypotension.
Uses & Indications
Dosage
Adults
Oral: 25-50 mg, 3-4 times daily. Maximum 150 mg/day.
Elderly
Lower doses recommended due to increased sensitivity and potential for adverse effects. Initiate with 12.5-25 mg, 2-3 times daily.
Renal_impairment
Use with caution; dose adjustment may be necessary. Monitor for accumulation.
How to Take
Take orally with a glass of water, with or without food. Avoid taking late in the evening to prevent insomnia.
Mechanism of Action
Ephidin acts as a direct and indirect adrenergic agonist, stimulating both alpha and beta receptors. It causes the release of norepinephrine from sympathetic nerve endings, leading to bronchodilation, vasoconstriction, and increased heart rate and blood pressure.
Pharmacokinetics
Onset
Oral: 30-60 minutes; Intravenous: rapid.
Excretion
Primarily excreted unchanged in the urine; excretion rate is pH-dependent (faster in acidic urine).
Half life
Approximately 3-6 hours.
Absorption
Well absorbed orally, with effects observed within 15-60 minutes.
Metabolism
Partially metabolized in the liver by N-demethylation; largely excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to ephidin or other sympathomimetics
- Narrow-angle glaucoma
- Severe hypertension or uncontrolled cardiovascular disease
- Pheochromocytoma
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs)
Drug Interactions
Beta-blockers
May reduce the bronchodilator effects of ephidin and may lead to unopposed alpha-adrenergic effects (hypertension).
Alpha-adrenergic blockers
May reduce the vasoconstrictive effects of ephidin.
Tricyclic Antidepressants (TCAs)
May potentiate the pressor effects of ephidin.
Monoamine Oxidase Inhibitors (MAOIs)
Concurrent use can lead to hypertensive crisis (severe increase in blood pressure). Avoid ephidin for at least 14 days after stopping an MAOI.
Storage
Store in a cool, dry place away from direct sunlight and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose include severe tachycardia, arrhythmias, hypertension, anxiety, tremor, seizures, delirium, and coma. Management is primarily supportive, including gastric lavage, activated charcoal, and symptomatic treatment for cardiovascular and CNS effects (e.g., benzodiazepines for seizures, alpha-blockers for severe hypertension).
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit outweighs the potential risk to the fetus. Ephidin is excreted in breast milk; caution is advised during breastfeeding.

Clinical Trials
As a very old drug, modern clinical trials for new indications are rare. Most evidence is historical or from specific niche uses (e.g., anesthesia-induced hypotension).
Lab Monitoring
- Blood pressure and heart rate (regularly)
- Electrolyte levels (in severe cases or overdose)
- Renal function (in patients with impairment)
Doctor Notes
- Careful titration is required, especially in elderly or cardiovascularly compromised patients.
- Monitor blood pressure and heart rate closely during treatment.
- Educate patients on potential for abuse and adverse effects, especially insomnia and nervousness.
- Avoid co-administration with MAOIs; allow a washout period.
Patient Guidelines
- Do not exceed the prescribed dose or frequency.
- Avoid taking ephidin close to bedtime to prevent sleep disturbances.
- Report any severe chest pain, shortness of breath, or palpitations immediately.
- Do not use with other decongestants or cold medicines without consulting a doctor.
- Be aware of potential for misuse due to stimulant effects.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Ephidin may cause dizziness, nervousness, or blurred vision in some individuals. Patients should assess their response to the medication before driving or operating machinery.
Lifestyle Advice
- Avoid or limit alcohol consumption.
- Minimize intake of caffeine and other stimulants.
- Maintain a balanced diet and regular exercise routine.
