Description
Overview of the medicine
Epicet-XR 1000 mg Tablet contains extended-release Paracetamol, used for the relief of mild to moderate pain and to reduce fever. The extended-release formulation provides prolonged pain relief.
Uses & Indications
Dosage
Adults
One 1000 mg tablet every 6-8 hours as needed. Do not exceed 3000 mg (3 tablets) in 24 hours.
Elderly
Generally, same as adult dose, but caution is advised. Consult a doctor for appropriate dosage.
Renal_impairment
Dosage adjustment may be necessary in severe renal impairment. Consult a doctor.
How to Take
Take the tablet orally with water. Do not crush, chew, or break the extended-release tablet, as this may lead to rapid release and absorption of a potentially toxic dose.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system by inhibiting prostaglandin synthesis, which reduces pain and fever. Its exact mechanism is not fully understood, but it may also involve interaction with the serotonergic system.
Pharmacokinetics
Onset
Onset of action for pain relief is typically 30-60 minutes, with prolonged effect due to XR formulation (up to 8 hours or more).
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma half-life is typically 2-3 hours for immediate release, but extended with XR formulations (up to 4-5 hours or more).
Absorption
Well absorbed from the gastrointestinal tract, though absorption rate may be slower with extended-release formulations.
Metabolism
Primarily metabolized in the liver by conjugation with glucuronide and sulfate. A small amount is metabolized by cytochrome P450 to a toxic intermediate (NAPQI) which is detoxified by glutathione.
