Description
Overview of the medicine
Epomax 5000 IU Injection contains Epoetin Alfa, a recombinant human erythropoietin, used to treat anemia associated with chronic kidney disease, chemotherapy, or certain surgical procedures by stimulating red blood cell production.
Uses & Indications
Dosage
Adults
Dosage varies significantly based on indication, patient weight, and response. For CKD anemia, initial dose typically 50-100 IU/kg three times weekly (IV or SC).
Elderly
No specific dose adjustment required, but monitor carefully for adverse effects.
Renal_impairment
Used to treat anemia associated with renal impairment; dosage individualized based on response.
How to Take
Administered intravenously (IV) or subcutaneously (SC). IV administration is typically for patients on hemodialysis. SC administration is preferred for most other patients.
Mechanism of Action
Epoetin alfa stimulates erythropoiesis by acting as an erythropoietin analogue. It binds to erythropoietin receptors on erythroid progenitor cells, promoting their proliferation and differentiation into mature red blood cells.
Pharmacokinetics
Onset
Hemoglobin increase typically observed within 2-6 weeks.
Excretion
Primarily excreted via catabolism; minimal renal excretion of intact drug.
Half life
Approximately 4-13 hours after IV administration, and 12-28 hours after SC administration.
Absorption
Bioavailability is approximately 30-40% after subcutaneous administration; nearly 100% after intravenous administration.
Metabolism
Primarily catabolized by erythroid progenitor cells and non-erythroid cells.
Side Effects
Contraindications
- •Uncontrolled hypertension.
- •Pure red cell aplasia (PRCA) that begins after treatment with Epoetin alfa or other erythropoietin proteins.
- •Serious allergic reactions to Epoetin alfa or any excipients.
Drug Interactions
Other hematopoietic growth factors
Can augment erythropoietic response.
No significant direct drug-drug interactions
No significant direct drug-drug interactions are known for epoetin alfa.
Storage
Store in a refrigerator at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light. Do not shake.
Overdose
Overdose can lead to polycythemia (excessive red blood cells), which may require phlebotomy to reduce blood viscosity. Supportive care should be provided.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. It is unknown whether Epoetin alfa is excreted in human milk; caution should be exercised when administered to a nursing woman.
