Description
Overview of the medicine
Eraxit 0.5 mg Tablet contains Bromazepam, a benzodiazepine primarily used for the short-term treatment of anxiety, panic attacks, and sleep disorders. It works by enhancing the effects of a natural brain chemical (GABA) that calms nerve activity.
Uses & Indications
Dosage
Adults
Typical oral dose: 1.5-3 mg, 2-3 times daily. Max 6 mg 3 times daily in severe cases. For anxiety, usually 3-6 mg daily in divided doses. For severe cases, up to 18 mg daily in divided doses.
Elderly
Lower doses are recommended, typically half of the adult dose (e.g., 1.5 mg daily in divided doses), adjusted based on response and tolerability.
Renal_impairment
Caution advised; dose reduction may be necessary. Start with lower doses.
How to Take
Take orally with water, with or without food. Do not chew or crush the tablet. Take exactly as prescribed by your doctor. Do not stop abruptly.
Mechanism of Action
Bromazepam binds to benzodiazepine receptors on GABA-A neurons in the central nervous system, enhancing the inhibitory effects of GABA. This leads to reduced neuronal excitability, resulting in anxiolytic, sedative, hypnotic, muscle relaxant, and anticonvulsant properties.
Pharmacokinetics
Onset
30-60 minutes
Excretion
Mainly excreted in the urine as metabolites.
Half life
Approximately 10-20 hours (variable, can be up to 30 hours)
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP450 enzymes (mainly CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to bromazepam or other benzodiazepines
- Severe respiratory insufficiency
- Severe hepatic insufficiency
- Sleep apnoea syndrome
- Myasthenia gravis
Drug Interactions
Alcohol
Potentiates sedative effects, increased risk of CNS depression.
Opioids
Increased risk of sedation, respiratory depression, coma, and death.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
May increase bromazepam plasma levels.
Other CNS depressants (e.g., antidepressants, antipsychotics, antihistamines)
Increased sedative effects.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, confusion, lethargy, ataxia, dysarthria, and hypotonia. In severe cases, respiratory depression, hypotension, and coma may occur. Management involves supportive care, gastric lavage if recent ingestion, and potentially flumazenil (a benzodiazepine antagonist) under strict medical supervision.
Pregnancy & Lactation
Pregnancy: Not recommended during pregnancy, especially during the first trimester, due to potential risk of congenital malformations. Use only if clearly needed and the potential benefits outweigh the risks. Lactation: Bromazepam passes into breast milk. Not recommended for use during breastfeeding due to potential sedation and feeding difficulties in the infant.
