Overview of the medicine
Esipram 5 mg Tablet contains Escitalopram, a selective serotonin reuptake inhibitor (SSRI) used to treat depression and anxiety disorders. It helps restore the balance of a certain natural substance (serotonin) in the brain.
Uses & Indications
Dosage
Adults
Initial dose 10 mg once daily, may be increased to a maximum of 20 mg once daily after at least one week. For 5 mg tablet, it's often a starting dose or for specific conditions.
Elderly
Initial dose 5 mg once daily, maximum recommended dose 10 mg once daily.
Renal_impairment
No dosage adjustment generally required for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water. Do not chew or crush.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing its neurotransmission.

Onset
Therapeutic effects may take 2-4 weeks to become evident.
Excretion
Mainly via urine (renal) and feces (hepatic).
Half life
Approximately 27-32 hours.
Absorption
Well absorbed orally, independent of food. Peak plasma concentration reached in approximately 4 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2C19, CYP3A4, and CYP2D6 to its major metabolites, S-demethylcitalopram and S-didesmethylcitalopram.
Side Effects
Contraindications
- Hypersensitivity to escitalopram or citalopram
- Concomitant use with MAOIs (monoamine oxidase inhibitors) or within 14 days of discontinuing an MAOI
- Concomitant use with pimozide
- Congenital long QT syndrome or known acquired long QT interval prolongation
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
Pimozide
Contraindicated due to risk of QT prolongation.
Warfarin
Altered anticoagulant effects, increased bleeding risk.
SSRIs/SNRIs
Increased risk of serotonin syndrome.
NSAIDs, Aspirin
Increased risk of gastrointestinal bleeding.
Lithium, Tryptophan
Increased risk of serotonin syndrome.
Tricyclic Antidepressants (TCAs)
May increase plasma levels of TCAs, increasing risk of side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, tremor, nausea, vomiting, somnolence, sinus tachycardia, and QT interval prolongation. Treatment is symptomatic and supportive; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Escitalopram is excreted in breast milk; advise caution/consider alternatives during breastfeeding.
