Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that decreases the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), ulcers, and certain other stomach acid-related problems.
Uses & Indications
Dosage
Adults
20-40 mg once daily, depending on the indication. For GERD, typically 20 mg once daily for 4-8 weeks.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal function.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole with a glass of water at least one hour before a meal. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase in the gastric parietal cell. By acting specifically on the proton pump, esomeprazole blocks the final step in acid production, thus reducing gastric acidity.

Onset
Within 1 hour.
Excretion
Primarily excreted in urine (80%) and feces (20%) as metabolites.
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 50-60%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Diazepam
May prolong the elimination of diazepam due to CYP2C19 inhibition.
Warfarin
Increased INR and prothrombin time. Monitor closely.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19. Consider alternative antiplatelet therapy.
Methotrexate
May increase serum levels of methotrexate. Consider temporary withdrawal of esomeprazole during high-dose methotrexate therapy.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, nausea, diarrhea, and headache. Treatment is largely symptomatic and supportive. Esomeprazole is not dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Use with caution in nursing mothers as esomeprazole is excreted in human milk.
