Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat gastroesophageal reflux disease (GERD), ulcers, and other conditions involving excessive stomach acid.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 7-10 days (with appropriate antibiotics).
Elderly
No specific dose adjustment is needed for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Oral capsules should be swallowed whole with water, at least one hour before a meal. Do not chew or crush. IV injection is administered by healthcare professionals.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase enzyme system (proton pump) on the secretory surface of the gastric parietal cell, thereby blocking the final step of acid production.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Approximately 80% of a dose is excreted as metabolites in the urine and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 50% after a single 20 mg oral dose, increasing to 68% after repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole or any components of the formulation, or to other substituted benzimidazoles.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time.
Nelfinavir
Reduced nelfinavir levels.
Tacrolimus
May increase tacrolimus levels.
Clopidogrel
May reduce antiplatelet effect.
Methotrexate
May increase methotrexate levels, leading to toxicity.
