Overview of the medicine
Esosav 20 mg Capsule is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For erosive esophagitis: 40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 7-10 days (with antibiotics).
Elderly
No dosage adjustment generally required.

How to Take
Take capsule whole with water at least one hour before a meal. Do not chew or crush.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. This inhibition prevents the final step of acid secretion, leading to a reduction in gastric acidity.

Acid suppression typically begins within 1 hour after administration.
Excretion
Primarily excreted via urine (approximately 80%) and feces (approximately 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration is reached within 1-2 hours. Bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications

Drug Interactions
Warfarin
Increased INR and prothrombin time. Monitor closely.
Clopidogrel
Reduced effectiveness of clopidogrel. Consider alternative antiplatelet therapy.
Methotrexate
Increased methotrexate levels. Monitor for toxicity.
Nelfinavir, Atazanavir
Reduced plasma levels of antivirals. Avoid concomitant use.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, nausea, diarrhea, and headache. Treatment is symptomatic and supportive.
