Description
Overview of the medicine
Esosic 20 mg Capsule contains Esomeprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is effective in treating conditions like GERD, ulcers, and other acid-related digestive issues.
Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, depending on the condition. For GERD, 20-40 mg daily for 4-8 weeks. For H. pylori eradication, 20 mg twice daily for 7-14 days.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is needed for patients with renal impairment.
How to Take
Take the capsule whole with water at least one hour before a meal. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step in acid secretion and reducing gastric acidity.
Pharmacokinetics
Onset
Acid secretion inhibition occurs within 1 hour, with full therapeutic effect achieved within 2-3 days of daily dosing.
Excretion
Approximately 80% of the dose is excreted as metabolites in the urine, and the remaining 20% in the feces.
Half life
Plasma elimination half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours. Bioavailability is approximately 50-68% (first dose) and increases with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin (caution in elderly or renal impaired patients).
Warfarin
Increased INR and prothrombin time, leading to increased risk of bleeding.
Tacrolimus
Increased systemic exposure of tacrolimus.
Clopidogrel
Reduced antiplatelet effect of clopidogrel, increasing cardiovascular risk.
Methotrexate
Increased and prolonged elevation of serum methotrexate and/or its metabolite, leading to potential toxicity.

Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
There is limited experience with overdose. Management should be symptomatic and supportive. Gastric lavage may be considered.
