Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that effectively reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For erosive esophagitis: 40 mg once daily for 4-8 weeks. For H. pylori eradication: 40 mg once daily with appropriate antibiotics for 10 days.
Elderly
No dosage adjustment is generally required for elderly patients, but caution should be exercised in those with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment. For severe renal impairment, monitor with caution.
How to Take
Take the capsule whole with water, at least one hour before a meal. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (the proton pump) in the parietal cells of the stomach. This action blocks the final step in gastric acid secretion, significantly reducing acid production.

Excretion
Mainly excreted via urine (approximately 80%) and feces (approximately 20%).

Approximately 1-1.5 hours.
Absorption

Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation
- Concomitant use with nelfinavir
Drug Interactions
Diazepam
May decrease the metabolism of diazepam.
Warfarin
May increase INR and prothrombin time; close monitoring is recommended.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel by inhibiting its conversion to active metabolite.
Methotrexate
May increase plasma concentrations of methotrexate; consider temporary withdrawal of PPI.
Nelfinavir/Atazanavir
Decreased plasma concentrations of antiretrovirals; concomitant use is contraindicated with nelfinavir.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include drowsiness, tachycardia, nausea, diarrhea, headache. Treatment should be symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed and potential benefits outweigh risks. It is unknown whether esomeprazole is excreted in human milk; use with caution in breastfeeding mothers.
