Description
Overview of the medicine
Esovir 20 mg Tablet is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and other acid-related issues.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4 to 8 weeks. For PUD: 20-40 mg once daily. For H. pylori eradication: 20 mg twice daily in combination with antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take Esovir 20 mg Tablet orally, usually once daily, at least 30-60 minutes before a meal with a glass of water. Do not chew or crush the tablet; swallow it whole.
Mechanism of Action
Esovir works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (proton pump) in the parietal cells of the stomach, which is the final step in gastric acid secretion, thereby reducing acid production.
Pharmacokinetics
Onset
Onset of action typically occurs within 1-2 hours.
Excretion
Metabolites are primarily excreted in the urine (approximately 80%) and in the feces (approximately 20%).
Half life
Plasma elimination half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed, with an approximate bioavailability of 50-60%.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Esovir, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with atazanavir or nelfinavir due to potential reduction in antiviral efficacy.
Drug Interactions
Digoxin
May increase the absorption and serum concentrations of digoxin; monitor digoxin levels.
Iron Salts
May decrease the absorption of iron salts due to reduced gastric acidity.
Tacrolimus
May increase tacrolimus serum concentrations; monitor tacrolimus levels.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel; concomitant use is generally discouraged.
Methotrexate
May increase methotrexate plasma levels, potentially leading to toxicity. Dose adjustment or temporary discontinuation of Esovir may be needed.
Storage
Store below 25°C in a dry and cool place. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for Esovir. Hemodialysis is not effective in removing the drug.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Exercise caution when administered to a nursing woman as it may be excreted in breast milk.

WHO Essential Medicine
YesClinical Trials
The efficacy and safety of proton pump inhibitors like Esovir (assuming it's analogous to Esomeprazole) have been established in numerous clinical trials for the treatment of GERD, PUD, and H. pylori eradication.
Lab Monitoring
- Monitor serum magnesium levels periodically in patients expected to be on prolonged treatment or who take concomitant drugs such as diuretics or digoxin.
- Monitor liver function tests in patients with severe hepatic impairment.
Doctor Notes
- Counsel patients on the potential risks associated with long-term PPI therapy, especially for bone health and C. difficile infection.
- Be aware of potential drug interactions, particularly with clopidogrel and methotrexate, and adjust therapy as necessary.
- Investigate any 'alarm symptoms' (e.g., dysphagia, unexplained weight loss, GI bleeding) before initiating PPI therapy.
Patient Guidelines
- Take this medicine exactly as prescribed by your doctor.
- Swallow the tablet whole with water; do not chew, crush, or break it.
- Take it at least 30-60 minutes before your first meal of the day.
- Inform your doctor if your symptoms do not improve or worsen.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Esovir is generally not expected to affect the ability to drive or operate machinery. However, if you experience dizziness or visual disturbances, exercise caution.
Lifestyle Advice
- Avoid trigger foods such as spicy, fatty, or acidic foods, caffeine, and alcohol.
- Elevate the head of your bed to help reduce acid reflux symptoms at night.
- Maintain a healthy weight and quit smoking to improve symptoms.
