Overview of the medicine
Esovir 20 mg Tablet is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and other acid-related issues.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4 to 8 weeks. For PUD: 20-40 mg once daily. For H. pylori eradication: 20 mg twice daily in combination with antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take Esovir 20 mg Tablet orally, usually once daily, at least 30-60 minutes before a meal with a glass of water. Do not chew or crush the tablet; swallow it whole.
Mechanism of Action
Esovir works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (proton pump) in the parietal cells of the stomach, which is the final step in gastric acid secretion, thereby reducing acid production.

Onset
Onset of action typically occurs within 1-2 hours.
Excretion
Metabolites are primarily excreted in the urine (approximately 80%) and in the feces (approximately 20%).
Half life
Plasma elimination half-life is approximately 1-1.5 hours.

Rapidly absorbed, with an approximate bioavailability of 50-60%.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to Esovir, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with atazanavir or nelfinavir due to potential reduction in antiviral efficacy.
Drug Interactions
Digoxin
May increase the absorption and serum concentrations of digoxin; monitor digoxin levels.
Iron Salts
May decrease the absorption of iron salts due to reduced gastric acidity.
Tacrolimus
May increase tacrolimus serum concentrations; monitor tacrolimus levels.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel; concomitant use is generally discouraged.
Methotrexate
May increase methotrexate plasma levels, potentially leading to toxicity. Dose adjustment or temporary discontinuation of Esovir may be needed.
Storage
Store below 25°C in a dry and cool place. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for Esovir. Hemodialysis is not effective in removing the drug.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Exercise caution when administered to a nursing woman as it may be excreted in breast milk.
