Description
Overview of the medicine
Norethisterone is a synthetic progestogen, a derivative of testosterone, used for various hormonal indications including dysfunctional uterine bleeding, endometriosis, premenstrual syndrome, postponement of menstruation, and as a component of hormone replacement therapy or oral contraception.
Uses & Indications
Dosage
Adults
Dosage varies significantly by indication: - Dysfunctional Uterine Bleeding: 5-10 mg daily for 5-10 days to stop bleeding; for prevention, 5 mg daily from day 5-25 of cycle. - Endometriosis: 10-20 mg daily continuously for 4-9 months. - Postponement of Menstruation: 5 mg three times daily, starting 3 days before expected period, for a maximum of 10-14 days. - HRT (to oppose estrogen): 1-5 mg daily continuously or sequentially (e.g., 5-10 mg daily for 12-14 days per 28-day cycle). - Contraception: Usually 0.35 mg as a progestogen-only pill; for 5 mg formulations, off-label for specific uses under strict medical guidance.
Elderly
Use with caution due to potential age-related decrease in hepatic function. No specific dose adjustment generally required, but monitor for adverse effects.
Renal_impairment
No specific dose adjustment is generally required, but use with caution in severe renal impairment.
How to Take
Ethinor-5 mg tablet should be taken orally with water, with or without food, as directed by a physician. It is best to take it at the same time each day.
Mechanism of Action
Norethisterone mimics the actions of natural progesterone by binding to progesterone receptors. Its effects vary depending on the dose and indication, including inducing secretory changes in the endometrium, suppressing ovulation (at higher doses), changing cervical mucus consistency, and influencing gonadotropin secretion.
Pharmacokinetics
Onset
Onset of action varies by indication; contraceptive effect within days, withdrawal bleeding typically occurs within 2-4 days of cessation.
Excretion
Approximately 60% of a dose is excreted in the urine and 40% in the feces as metabolites.
Half life
Plasma elimination half-life ranges from 5 to 12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily via reduction and conjugation. Significant first-pass metabolism occurs. Metabolites are excreted as glucuronide and sulfate conjugates.
Side Effects
Contraindications
- •Known or suspected pregnancy
- •Undiagnosed vaginal bleeding
- •History of thromboembolic disorders (e.g., deep vein thrombosis, pulmonary embolism)
- •Severe liver disease or liver tumors
- •Known or suspected breast cancer, or other progestogen-sensitive cancers
- •Hypersensitivity to norethisterone or any component of the tablet
Drug Interactions
Cyclosporine
Norethisterone may increase plasma concentrations of cyclosporine.
Anticoagulants
Norethisterone may alter the effects of anticoagulants; dose adjustment may be necessary.
Certain Antibiotics (e.g., Tetracyclines, Amoxicillin)
May reduce the contraceptive efficacy of progestogen-only pills (though evidence is inconclusive, backup contraception is often advised).
Enzyme Inducers (e.g., Rifampicin, Carbamazepine, Phenytoin, St. John's Wort)
May decrease the plasma concentration and efficacy of norethisterone.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include nausea, vomiting, breast tenderness, dizziness, and vaginal bleeding. Treatment is symptomatic and supportive; serious effects are unlikely.
Pregnancy & Lactation
Norethisterone is contraindicated during pregnancy due to potential risks to the fetus. It passes into breast milk and is generally not recommended during lactation, especially in the first 6 weeks postpartum.
