Description
Overview of the medicine
Etnaaf 90 mg Tablet contains Etoricoxib, a non-steroidal anti-inflammatory drug (NSAID) that selectively inhibits cyclooxygenase-2 (COX-2) enzyme. It is used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and acute gouty arthritis.
Uses & Indications
Dosage
Adults
For Rheumatoid Arthritis and Ankylosing Spondylitis: 90 mg once daily. For acute pain/gout: 120 mg once daily for a maximum of 8 days, or as directed by physician.
Elderly
No specific dose adjustment is required for the elderly, but caution should be exercised due to potential age-related decrease in renal function.
Renal_impairment
For mild to moderate renal impairment, no dose adjustment needed. For severe renal impairment, Etoricoxib is not recommended.
How to Take
Take Etnaaf 90 mg Tablet orally, once daily. It can be taken with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Etoricoxib is a selective cyclooxygenase-2 (COX-2) inhibitor. By inhibiting COX-2, it reduces the synthesis of prostaglandins, which are mediators of pain and inflammation. This results in anti-inflammatory, analgesic, and antipyretic effects.
Pharmacokinetics
Onset
Onset of action for pain relief generally occurs within 30-60 minutes.
Excretion
Approximately 70% is excreted in the urine and 20% in the feces, mainly as metabolites.
Half life
The elimination half-life is approximately 22 hours.
Absorption
Rapidly absorbed orally, with peak plasma concentrations reached in approximately 1 hour.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 (CYP) enzymes, particularly CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Etoricoxib or any component of the tablet
- Active peptic ulceration or gastrointestinal (GI) bleeding
- Severe hepatic dysfunction or severe renal impairment
- Inflammatory bowel disease
- Congestive heart failure (NYHA Class II-IV)
- Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
Drug Interactions
Lithium
May increase plasma lithium levels and toxicity.
Warfarin
Increased prothrombin time (INR) with concomitant use, requiring monitoring.
Diuretics
May reduce natriuretic effect.
Rifampicin
May decrease Etoricoxib plasma concentrations.
Methotrexate
May increase plasma concentrations of methotrexate, increasing toxicity.
ACE inhibitors/Angiotensin II Receptor Antagonists
May reduce the antihypertensive effect and increase the risk of renal impairment.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for Etoricoxib.
Pregnancy & Lactation
Etoricoxib should be avoided during pregnancy, especially in the third trimester due to potential premature closure of the ductus arteriosus and effects on fetal renal function. It is not known whether Etoricoxib is excreted in human milk, so it should be avoided during lactation.
