Description
Overview of the medicine
Evrysdi (risdiplam) is an oral medication approved for the treatment of Spinal Muscular Atrophy (SMA) in adults and children 2 months of age and older. It works by increasing and sustaining the production of the survival motor neuron (SMN) protein.
Uses & Indications
Dosage
Adults
Weight-based dosing. Typically 0.25 mg once daily for patients <20 kg, 0.5 mg once daily for patients ≥20 kg but <40 kg, and 0.6 mg once daily for patients ≥40 kg.
Elderly
No specific dose adjustment recommended based on age alone.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Data limited for severe renal impairment.
How to Take
Administer orally once daily after a meal, at approximately the same time each day. Can be given via nasogastric or gastrostomy tube if necessary. Must be reconstituted before use.
Mechanism of Action
Risdiplam is a survival motor neuron 2 (SMN2) splicing modifier that is designed to increase and sustain the production of functional survival motor neuron (SMN) protein in the body.

Excretion
Mainly through feces (53%), with a smaller portion excreted renally (14%).
Half life
Approximately 50 hours in adults, 48 hours in infants/children.
Absorption
Rapidly absorbed, peak plasma concentration reached in 1-4 hours.

Primarily by flavin-containing monooxygenase (FMO) 1 and 3, and to a lesser extent by CYP enzymes.

Drug Interactions
P-gp substrates
Risdiplam is a P-gp substrate. Co-administration with P-gp inhibitors/inducers may alter risdiplam exposure.
CYP1A2 substrates
Risdiplam may inhibit CYP1A2. Use caution with co-administration of CYP1A2 substrates with narrow therapeutic index.
Storage
Unopened bottle: Store at room temperature (20°C to 25°C). Reconstituted solution: Store at room temperature (20°C to 25°C) or refrigerated (2°C to 8°C). Do not freeze.
Overdose
No specific antidote. Symptomatic and supportive treatment is recommended. Monitor for signs of adverse reactions.
Pregnancy & Lactation
Pregnancy: May cause fetal harm. Advise females of reproductive potential to use effective contraception. Lactation: Not known if excreted in human milk. Advise against breastfeeding.
