Overview of the medicine
Exalin 10 mg Tablet is an antidepressant medication primarily used for the treatment of major depressive disorder and various anxiety disorders. It works by increasing the levels of serotonin in the brain, a natural substance that helps maintain mental balance.
Uses & Indications
Dosage
Adults
Initial dose 10 mg once daily, may be increased to a maximum of 20 mg once daily after 1-2 weeks based on response.
Elderly
Initial dose 5 mg once daily, maximum 10 mg once daily. Adjust with caution.
Renal_impairment
No dose adjustment generally required for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Exalin selectively inhibits the reuptake of serotonin (5-HT) by neurons in the central nervous system. This increases the concentration of serotonin in the synaptic cleft, leading to enhanced serotonergic neurotransmission.

Onset
Initial therapeutic effects typically observed within 1-2 weeks, full effect in 4-6 weeks.
Excretion
Mainly excreted via urine, with a small portion in feces.
Half life
Approximately 24-36 hours.
Absorption
Well absorbed orally, with peak plasma concentrations reached in 4-6 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP2D6 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Exalin or any excipients
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy
- •Concomitant use with pimozide
Drug Interactions
MAOIs
Concurrent use may lead to serious, sometimes fatal, reactions including Serotonin Syndrome.
NSAIDs
Increased risk of gastrointestinal bleeding.
Warfarin
Increased risk of bleeding due to potentiation of anticoagulant effect.
Tricyclic Antidepressants (TCAs)
Increased plasma concentrations of TCAs, requiring dose adjustment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, tremor, nausea, vomiting, somnolence, and tachycardia. In severe cases, seizures and coma may occur. Management involves supportive care, maintaining airway, and monitoring vital signs. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as Exalin is excreted in breast milk.
