Description
Overview of the medicine
Exib 60 mg Tablet contains Etoricoxib, a selective COX-2 inhibitor, used for relieving pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis. It is also used for the short-term treatment of moderate pain following dental surgery.
Uses & Indications
Dosage
Adults
Osteoarthritis: 30 mg or 60 mg once daily. Rheumatoid Arthritis, Ankylosing Spondylitis: 60 mg or 90 mg once daily. Acute Gouty Arthritis: 120 mg once daily for a maximum of 8 days. Acute Pain: 60 mg once daily, maximum for 8 days.
Elderly
No dosage adjustment generally required, but caution is advised. Initiate at the lowest effective dose.
Renal_impairment
Mild to moderate (CrCl 30-80 mL/min): No dosage adjustment. Severe (CrCl <30 mL/min): Not recommended.
How to Take
Take orally, with or without food. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Etoricoxib is a non-steroidal anti-inflammatory drug (NSAID) that selectively inhibits cyclooxygenase-2 (COX-2). COX-2 is responsible for the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. By inhibiting COX-2, Etoricoxib reduces prostaglandin production, thereby alleviating pain and inflammation.
Pharmacokinetics
Onset
Within 24 minutes for acute pain, within 1-2 hours for chronic conditions.
Excretion
Mainly excreted renally (approximately 70% as metabolites) and fecally (approximately 20% as metabolites).
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations reached in approximately 1 hour.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any excipients
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe hepatic impairment (Child-Pugh score >9)
- •Severe renal impairment (CrCl <30 mL/min)
- •Inflammatory bowel disease
- •Congestive heart failure (NYHA Class II-IV)
- •Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Previous history of bronchospasm, acute rhinitis, nasal polyps, angioedema, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Pregnancy and lactation
Drug Interactions
Lithium
Increased plasma lithium levels, increased lithium toxicity.
Warfarin
Increased prothrombin time/INR, increased bleeding risk.
Methotrexate
Increased plasma methotrexate concentrations, increased methotrexate toxicity.
Oral Contraceptives
Increased plasma concentrations of ethinylestradiol, potentially increasing adverse events associated with oral contraceptives.
Diuretics and ACE Inhibitors/Angiotensin II Antagonists
May reduce the natriuretic effect and antihypertensive effect, respectively. Risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote. Haemodialysis is unlikely to remove etoricoxib effectively.
Pregnancy & Lactation
Pregnancy Category C in the first and second trimesters, Category D in the third trimester. Avoid during pregnancy due to potential risk of premature closure of the ductus arteriosus and fetal renal dysfunction. Excreted in breast milk, therefore avoid during lactation.
