Description
Overview of the medicine
Exler 10 mg Tablet contains Escitalopram, an antidepressant used to treat major depressive disorder, generalized anxiety disorder, panic disorder, and other anxiety-related conditions.
Uses & Indications
Dosage
Adults
Initial: 10 mg once daily. May be increased to 20 mg once daily after at least one week, based on clinical response. Maximum: 20 mg/day.
Elderly
Initial: 5 mg once daily. Maximum recommended dose is 10 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the brain, thereby increasing the concentration of serotonin in the synaptic clefts, which enhances serotonergic neurotransmission and improves mood and reduces anxiety.
Pharmacokinetics
Onset
Initial antidepressant effects may be observed within 1-2 weeks, with full therapeutic benefits typically achieved in 4-6 weeks.
Excretion
Mainly renal excretion (approximately two-thirds) with some fecal excretion (one-third).
Half life
Approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with approximately 80% bioavailability. Peak plasma concentrations are reached in about 3-5 hours.
Metabolism
Primarily hepatic via CYP2C19, CYP3A4, and CYP2D6 enzymes to inactive and less potent active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to escitalopram or citalopram
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy
- •Concomitant use with Pimozide due to risk of QTc prolongation
Drug Interactions
MAOIs
Concomitant use is contraindicated due to risk of serotonin syndrome.
Pimozide
Concomitant use is contraindicated due to increased risk of QTc prolongation.
NSAIDs and Aspirin
Increased risk of gastrointestinal bleeding.
Warfarin and other anticoagulants
Increased risk of bleeding. Monitor INR.
Serotonergic drugs (e.g., Triptans, Tramadol, Linezolid, other SSRIs)
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include dizziness, tremor, nausea, vomiting, somnolence, sinus tachycardia, and rarely seizures or serotonin syndrome. Treatment is supportive and symptomatic, with maintenance of an open airway, oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered soon after ingestion.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Escitalopram is excreted in human breast milk; advise caution during lactation and weigh the benefits of breastfeeding against potential risks to the infant.
