Overview of the medicine
Exmart 20 mg tablet contains Escitalopram, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, generalized anxiety disorder, panic disorder, social anxiety disorder, and obsessive-compulsive disorder.
Uses & Indications
Dosage
Adults
Initial dose 10 mg once daily, may be increased to 20 mg once daily after at least one week based on response. Maximum recommended dose is 20 mg/day.
Elderly
A lower initial dose of 5 mg once daily may be considered, with careful titration up to a maximum of 10 mg once daily.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe renal impairment (creatinine clearance <20 mL/min).
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, thereby enhancing and prolonging the effects of serotonin in the brain. This action is thought to be responsible for its antidepressant and anxiolytic effects.

Onset
Initial therapeutic effects may be seen within 2-4 weeks, with full effect in 4-6 weeks.
Excretion
Excreted through urine (primarily) and feces as metabolites.
Half life
Approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, independent of food. Peak plasma concentrations are reached in approximately 4-5 hours.
Metabolism
Extensively metabolized in the liver by CYP3A4, CYP2C19, and CYP2D6 to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Escitalopram or any component of the formulation.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of stopping MAOI treatment.
- Concomitant use with Pimozide.
- Congenital long QT syndrome or acquired long QT interval.
Drug Interactions
Pimozide
Concomitant use is contraindicated due to increased QT prolongation risk.
Metoprolol
May increase Metoprolol plasma levels, adjust Metoprolol dose if needed.
Warfarin and other anticoagulants
Increased risk of bleeding due to effects on platelet aggregation.
MAOIs (e.g., Phenelzine, Selegiline)
Risk of serious, sometimes fatal, reactions including serotonin syndrome.
NSAIDs (e.g., Ibuprofen) and Aspirin
Increased risk of gastrointestinal bleeding.
Other serotonergic drugs (e.g., Triptans, Tramadol, St. John's Wort)
Increased risk of serotonin syndrome.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, tremor, agitation, somnolence, nausea, vomiting, sinus tachycardia, and in rare cases, serotonin syndrome. Treatment is supportive and symptomatic; ensure airway patency, monitor cardiac rhythm and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to SSRIs late in the third trimester have developed complications. Excreted into breast milk; caution advised during breastfeeding.
