Description
Overview of the medicine
Exomi-Mups 20 mg Tablet is a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It treats conditions like heartburn, acid reflux, stomach ulcers, and certain esophageal problems.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment necessary for patients with impaired renal function.
How to Take
Take one tablet whole with water, at least one hour before a meal, preferably in the morning. Do not chew or crush the tablet.
Mechanism of Action
Esomeprazole inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, thereby blocking the final step of acid secretion and reducing gastric acidity.

Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 20%) as inactive metabolites.
Half life
Approximately 1-1.5 hours
Absorption
Rapidly absorbed, bioavailability approximately 64% after a single 40 mg dose, increasing to 89% after repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by cytochrome P450 (CYP) enzymes, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Diazepam
Decreased clearance of diazepam.
Warfarin
Increased INR and prothrombin time.
Tacrolimus
Increased tacrolimus serum concentrations.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma levels of methotrexate.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal disturbances and weakness. Management is symptomatic and supportive. Esomeprazole is highly protein-bound and not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Caution is advised during breastfeeding as esomeprazole is excreted in human milk.
