Description
Overview of the medicine
Exopan 20 mg Tablet contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions like heartburn, acid reflux, and stomach ulcers.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 10 days (in combination therapy).
Elderly
No dosage adjustment is generally required for elderly patients; however, caution is advised.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Take the tablet whole, at least one hour before a meal, preferably in the morning. Do not chew or crush the tablet.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby blocking the final step of acid production. This leads to a dose-dependent inhibition of both basal and stimulated gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour of administration.
Excretion
Approximately 80% of a dose is excreted as metabolites in the urine, and the remainder in the feces.
Half life
Approximately 1.0 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma levels reached in 1-2 hours. Bioavailability is about 64% after a single 40 mg dose and increases to 89% after repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Co-administration with nelfinavir.
Drug Interactions
Diazepam
Decreased clearance, prolonged half-life of diazepam.
Warfarin
Increased INR and prothrombin time.
Nelfinavir
Significant decrease in nelfinavir exposure (co-administration contraindicated).
Clopidogrel
Potential reduction in clopidogrel's antiplatelet effect.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, nausea, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing mother.
