Description
Overview of the medicine
Expreso 200 mg Syrup contains Ibuprofen, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, reduce fever, and decrease inflammation. It is commonly prescribed for conditions such as headache, menstrual pain, dental pain, and minor injuries.
Uses & Indications
Dosage
Adults
Oral: 200-400 mg every 4-6 hours as needed. Max 1200 mg/day (OTC) or 2400-3200 mg/day (prescription, for severe conditions). For 200 mg/5ml syrup, this means 5-10 ml every 4-6 hours.
Elderly
Lower doses may be required due to increased risk of side effects, especially GI and renal. Start with lowest effective dose.
Children
Oral: 5-10 mg/kg body weight every 6-8 hours, maximum 40 mg/kg/day. (Specific dosage for 200mg/5ml syrup should be consulted with a doctor or product leaflet).
Renal_impairment
Use with caution. Dosage reduction may be necessary. Avoid in severe renal impairment.
How to Take
Shake the bottle well before each use. Take with food or milk to reduce stomach upset. Use the provided measuring device for accurate dosing.
Mechanism of Action
Ibuprofen works by inhibiting the production of prostaglandins, which are chemicals in the body that contribute to pain, fever, and inflammation. It achieves this by blocking the cyclooxygenase (COX) enzymes, COX-1 and COX-2.
Pharmacokinetics
Onset
Pain relief within 30-60 minutes, fever reduction within 1-2 hours.
Excretion
Primarily excreted in the urine (about 90%) as metabolites and conjugates, with a small amount in feces.
Half life
Approximately 1.8-2.0 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, peak plasma concentrations typically within 1-2 hours.
Metabolism
Extensively metabolized in the liver (mainly via oxidation and conjugation) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Ibuprofen or other NSAIDs (e.g., aspirin), leading to asthma, urticaria, or allergic-type reactions
- Active peptic ulcer or gastrointestinal bleeding
- Severe heart failure, renal failure, or hepatic failure
- Third trimester of pregnancy
- Children under 3 months of age without medical supervision
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased plasma methotrexate levels and toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced efficacy and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from direct sunlight. Do not freeze. Keep out of reach of children. Discard any unused medicine after 28 days of opening.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, drowsiness, dizziness, headache, tinnitus, and nystagmus. Severe overdose can lead to GI bleeding, hypotension, respiratory depression, metabolic acidosis, renal failure, and coma. Treatment is supportive and symptomatic; gastric lavage or activated charcoal may be considered if recent ingestion. Monitor renal and hepatic function.
Pregnancy & Lactation
Pregnancy Category C (D in 3rd trimester). Avoid use in the third trimester due to potential premature closure of the fetal ductus arteriosus and other risks. Use in first two trimesters only if clearly needed and benefits outweigh risks. Small amounts are excreted in breast milk; generally considered compatible with breastfeeding at usual doses, but caution is advised.
