Overview of the medicine
ATV Eze is an angiotensin receptor blocker (ARB) used to treat high blood pressure and heart failure, and to reduce the risk of cardiovascular mortality in patients with left ventricular dysfunction after a myocardial infarction.
Uses & Indications
Dosage
Adults
For hypertension: Initial dose 50 mg once daily. May be increased to 100 mg once daily. For heart failure: Initial dose 12.5 mg once daily, titrate up to 50 mg once daily as tolerated.
Elderly
No dose adjustment generally required, but caution is advised in elderly patients with impaired renal function.
Renal_impairment
For patients with moderate to severe renal impairment (CrCl <30 mL/min), consider a lower initial dose of 25 mg once daily.
How to Take
Take orally once daily with or without food. Swallow the tablet whole with water.
Mechanism of Action
ATV Eze selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues, including vascular smooth muscle and the adrenal gland. This inhibition leads to vasodilation, decreased aldosterone secretion, reduced sodium and water retention, and decreased blood pressure.
Pharmacokinetics
Onset
Onset of action within 2 hours; full blood pressure lowering effect within 2-4 weeks.
Excretion
Primarily excreted in the feces (83%) and urine (13%) as unchanged drug.
Half life
Approximately 6-9 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached in 1-2 hours. Bioavailability is approximately 30-40%.
Metabolism
Undergoes minimal metabolism in the liver; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to ATV Eze or any component of the formulation.
- Pregnancy (especially during the second and third trimesters).
- Concomitant use with aliskiren in patients with diabetes or renal impairment.
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics
Increased risk of hypotension and renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May attenuate antihypertensive effect and increase risk of renal impairment.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include profound hypotension and tachycardia; bradycardia may occur. Treatment is supportive; administer intravenous fluids for hypotension and manage symptoms as appropriate. Hemodialysis is unlikely to remove ATV Eze.

Category D in pregnancy (risk of fetal injury/death). Not recommended during breastfeeding due to potential excretion into breast milk and adverse effects on the infant.
