Description
Overview of the medicine
Faslodex (fulvestrant) is an estrogen receptor antagonist indicated for the treatment of hormone receptor-positive breast cancer. It acts by binding to the estrogen receptor and causing its degradation, thereby inhibiting estrogen's effects on tumor growth.
Uses & Indications
Dosage
Adults
500 mg administered intramuscularly into the buttock (two 250 mg injections, one into each buttock) on Days 1, 15, 29, and once monthly thereafter.
Elderly
No specific dose adjustment required based on age.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. Data limited for severe renal impairment; use with caution.
How to Take
For intramuscular use only. Administer as two separate 5 mL injections, one into each buttock, slowly over 1-2 minutes. Do not inject into a blood vessel or nerve.
Mechanism of Action
Fulvestrant is an estrogen receptor downregulator (ERD) that binds to the estrogen receptor with high affinity, competitively inhibiting estrogen binding. It leads to degradation of the ER protein, reducing ER levels and inhibiting estrogen-dependent tumor cell growth.
Pharmacokinetics
Onset
Sustained estrogen receptor downregulation observed within days. Clinical response varies.
Excretion
Primarily in feces (approximately 90%) via biliary excretion, with minimal renal excretion (less than 1%).
Half life
Approximately 40 days.
Absorption
Slowly absorbed after intramuscular injection, reaching peak plasma concentrations in approximately 5-7 days. Bioavailability is not well-established for IM route.
Metabolism
Primarily hepatic, via cytochrome P450 (CYP) 3A4 and other non-CYP pathways.
Side Effects
Contraindications
- Hypersensitivity to fulvestrant or any component of the formulation.
- Pregnancy and lactation.
- Severe hepatic impairment.
Drug Interactions
Anticoagulants
Caution with concomitant use due to potential for increased bleeding risk (fulvestrant is IM).
CYP3A4 Inducers
Decreased fulvestrant exposure (e.g., rifampin, phenytoin).
CYP3A4 Inhibitors
Increased fulvestrant exposure (e.g., ketoconazole, clarithromycin).
Storage
Store in a refrigerator (2°C to 8°C). Do not freeze. Protect from light.
Overdose
No specific antidote. Management involves general supportive care.
Pregnancy & Lactation
Contraindicated in pregnancy (fetal harm). Discontinue breastfeeding during treatment and for 1 year after the last dose.
