Description
Overview of the medicine
Fastdol 325 mg Tablet is a medication containing Paracetamol, widely used to relieve mild to moderate pain and reduce fever. It works by blocking certain chemical messengers that cause pain and fever.
Uses & Indications
Dosage
Adults
For adults, 325-650 mg every 4-6 hours as needed, not exceeding 4000 mg (4 grams) in 24 hours.
Elderly
Similar to adults, but caution advised for elderly patients, especially those with hepatic or renal impairment.
Renal_impairment
For severe renal impairment (CrCl <10 mL/min), consider extending the dosing interval to 6-8 hours. Consult a doctor.
How to Take
Oral administration. Can be taken with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Paracetamol's exact mechanism is not fully understood, but it is believed to act predominantly in the central nervous system (CNS) by inhibiting prostaglandin synthesis, possibly via selective inhibition of cyclooxygenase-2 (COX-2) or through a novel COX-3 pathway. It also has an antipyretic effect by acting on the hypothalamic heat-regulating center.
Pharmacokinetics
Onset
Onset of analgesic effect usually within 30-60 minutes; antipyretic effect within 30 minutes.
Excretion
Mainly excreted by the kidneys as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma elimination half-life is typically 1 to 4 hours, prolonged in liver disease or overdose.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are typically achieved within 30-60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver, mainly by conjugation with glucuronide and sulfate. A minor but highly reactive metabolite (N-acetyl-p-benzoquinone imine, NAPQI) is formed by cytochrome P450, which is normally detoxified by glutathione.
Side Effects
Contraindications
- •Hypersensitivity to Paracetamol or any component of the formulation
- •Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Chronic heavy alcohol use increases the risk of liver toxicity with Paracetamol.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding, especially with chronic high doses.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose can lead to severe hepatotoxicity, potentially fatal. Symptoms include nausea, vomiting, abdominal pain, and sweating. Management involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Considered safe for use during pregnancy and lactation at recommended therapeutic doses. However, always consult a healthcare professional before use.
