Description
Overview of the medicine
Fastfit-1-w-gel is a topical analgesic and anti-inflammatory gel formulated to provide relief from localized pain and inflammation associated with musculoskeletal conditions such as sprains, strains, backache, and osteoarthritis. It combines the anti-inflammatory action of Diclofenac Diethylamine with the counter-irritant effects of Methyl Salicylate and Menthol, enhanced by Linseed Oil.
Uses & Indications
Dosage
Adults
Apply a small amount of gel (2-4 cm strip depending on the size of the affected area) to the affected skin surface 2 to 4 times daily and rub in gently.
Elderly
No specific dosage adjustment is required for elderly patients. Use as per adult dosage.
Renal_impairment
No dosage adjustment is necessary as systemic absorption is minimal.
How to Take
For external use only. Apply a thin layer to the affected area and rub gently until absorbed. Wash hands thoroughly after application, unless hands are the treated area. Avoid contact with eyes, mucous membranes, and open wounds or broken skin.
Mechanism of Action
Diclofenac Diethylamine, a non-steroidal anti-inflammatory drug (NSAID), inhibits cyclooxygenase (COX-1 and COX-2) enzymes, reducing prostaglandin synthesis and thereby alleviating pain and inflammation. Methyl Salicylate and Menthol act as counter-irritants, causing a sensation of warmth or coolness, which distracts from pain and increases local blood flow.
Pharmacokinetics
Onset
Pain relief may begin within 30 minutes of application.
Excretion
Systemically absorbed diclofenac metabolites are excreted via urine and bile. For topical application, excretion routes are less significant due to low systemic exposure.
Half life
The systemic half-life of diclofenac is approximately 1-2 hours, but local tissue concentration is maintained for longer periods. For other ingredients, local effects are dominant.
Absorption
Minimal systemic absorption after topical application; primarily acts locally. Diclofenac penetrates the skin to reach underlying inflamed tissues. Linseed oil may act as a penetration enhancer.
Metabolism
Diclofenac is primarily metabolized in the liver, but topical absorption leads to very low systemic levels, so systemic metabolism is negligible. Other components are also metabolized locally or minimally systemically.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, other NSAIDs, or any component of the gel.
- On broken, damaged, infected, or eczematous skin.
- In patients with asthma, urticaria, or acute rhinitis precipitated by aspirin or other NSAIDs.
- Third trimester of pregnancy.
Drug Interactions
Oral NSAIDs
Although systemic absorption is low, concomitant use with oral NSAIDs may increase the risk of systemic side effects.
Anticoagulants
High-dose, prolonged use of topical diclofenac could theoretically enhance the effects of anticoagulants, though the risk is minimal.
Diuretics and Antihypertensives
May reduce the efficacy of diuretics and antihypertensive medications with significant systemic absorption, which is rare for topical application.
Storage
Store below 30°C in a dry place, protected from light and moisture. Do not freeze.
Overdose
Topical overdose is unlikely to lead to systemic toxicity due to minimal absorption. Accidental ingestion may cause gastrointestinal disturbances, nausea, vomiting. Management should be symptomatic and supportive.
Pregnancy & Lactation
Consult a doctor before use during pregnancy and lactation. Avoid during the third trimester of pregnancy due to potential risks to the fetus (e.g., premature closure of the ductus arteriosus). Use with caution during lactation; it is unknown if components are excreted in breast milk in significant amounts.
