Description
Overview of the medicine
Faxilin 125 mg Suspension is a first-generation cephalosporin antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, bone and joint infections, and urinary tract infections. It is generally well-tolerated and effective against a broad range of susceptible bacteria.
Uses & Indications
Dosage
Adults
Adults (though 125mg suspension is mostly for pediatric use): 250 mg to 1 g every 6-12 hours, depending on infection severity. Higher volumes of 125mg suspension would be needed.
Children
Children: 25-50 mg/kg/day in divided doses (e.g., every 6-8 hours). For severe infections, the dosage may be doubled. Do not exceed 4g/day. For a 10 kg child, this would be 6.25-12.5 mL of 125 mg/5 mL suspension every 6 hours.
Renal_impairment
Dosage should be reduced in patients with significant renal impairment (e.g., creatinine clearance < 50 mL/min). Consult a physician for specific adjustments.
How to Take
Oral administration. Can be taken with or without food. Taking with food may reduce gastrointestinal upset. Shake the bottle well before each use. Use the provided measuring spoon/cup for accurate dosing. Complete the full course as prescribed.
Mechanism of Action
Cephalexin works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Typically within 1 hour of oral administration.
Excretion
Primarily by the kidneys via glomerular filtration and tubular secretion (about 80% to 90% unchanged in urine within 8 hours).
Half life
Approximately 0.5 to 1.2 hours in adults with normal renal function, prolonged in renal impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, even in the presence of food. Peak plasma concentrations are achieved within 1 hour.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cephalexin or other cephalosporins
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to penicillins, due to potential cross-reactivity.
Drug Interactions
Metformin
Increased plasma concentrations of metformin and decreased renal clearance of metformin.
Probenecid
May increase and prolong plasma levels of cephalexin by inhibiting its renal excretion.
Aminoglycosides
Increased risk of nephrotoxicity when co-administered, especially in patients with pre-existing renal impairment.
Other nephrotoxic drugs
Concomitant use with other potentially nephrotoxic drugs should be monitored carefully.
Oral anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect by altering gut flora and reducing vitamin K synthesis, requiring INR monitoring.
Storage
Store dry powder below 30°C, in a cool, dry place, away from direct sunlight and moisture. Keep the bottle tightly closed. After reconstitution, store refrigerated at 2-8°C. Do not freeze. Discard any unused portion after the specified period (e.g., 7 or 14 days).
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, diarrhea, and hematuria. In very rare instances, overdose may lead to convulsions. Treatment is primarily supportive. Gastric lavage may be considered. Hemodialysis may be useful in severe cases, particularly if renal function is impaired.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy, but use only if clearly needed. Cephalexin is excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential adverse effects (e.g., diarrhea, candidiasis, sensitization).

WHO Essential Medicine
YesClinical Trials
Cephalexin has been extensively studied since its introduction and its efficacy and safety profile are well-established through numerous clinical trials. It continues to be a cornerstone antibiotic for many common bacterial infections, with ongoing studies confirming its role in resistance management and specific populations.
Lab Monitoring
- Renal function tests (e.g., creatinine, BUN) in patients with impaired renal function or on prolonged therapy
- Liver function tests (rarely needed for routine use, but may be monitored in patients with hepatic impairment or signs of liver dysfunction)
- Complete blood count (CBC) for prolonged therapy or if hematological abnormalities are suspected
Doctor Notes
- Verify patient's penicillin allergy history due to potential cross-reactivity.
- Emphasize the importance of completing the full course of therapy to prevent resistance.
- Consider renal dose adjustment in patients with impaired renal function.
- Counsel parents on accurate measurement and proper storage of the suspension.
Patient Guidelines
- Complete the full course of antibiotics as prescribed, even if symptoms improve, to prevent the development of antibiotic resistance and ensure complete eradication of the infection.
- Do not save leftover medicine for future infections.
- Shake the suspension well before each use to ensure even distribution of the medication.
- Measure dose accurately using the provided measuring spoon or cup to avoid under- or overdosing.
- Inform your doctor if you experience severe diarrhea or any signs of an allergic reaction.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Cephalexin generally does not affect the ability to drive or operate machinery. However, if you experience side effects such as dizziness, headache, or confusion, avoid such activities until you feel well.
Lifestyle Advice
- Maintain good personal hygiene to prevent reinfection, especially in urinary tract infections.
- Stay hydrated by drinking plenty of fluids.
- Consume probiotics (e.g., yogurt) to help maintain healthy gut flora, especially if experiencing diarrhea.
- Avoid self-medication and always consult a healthcare professional before starting or stopping any medication.
