Description
Overview of the medicine
Ephedrine is a sympathomimetic amine used as a stimulant, appetite suppressant, concentration aid, decongestant, and to treat hypotension, especially during spinal or general anesthesia.
Uses & Indications
Dosage
Adults
For hypotension during anesthesia: 5-10 mg intravenously (IV), repeated if necessary, up to a maximum of 50 mg/dose or 150 mg/day. Can also be given intramuscularly (IM) or subcutaneously (SC).
Elderly
Start at the lower end of the adult dose range and monitor closely due to increased sensitivity and potential for adverse effects.
Renal_impairment
Use with caution; dosage adjustment may be needed due to primary renal excretion. Monitor renal function.
How to Take
Administer intravenously (slowly), intramuscularly, or subcutaneously. The IV route is preferred for rapid onset and controlled titration during acute hypotension.
Mechanism of Action
Ephedrine acts directly on alpha and beta adrenergic receptors and indirectly by causing the release of noradrenaline from sympathetic neurons.

Onset
IV: immediate; IM/SC: 10-20 minutes.
Excretion
Primarily excreted unchanged in urine, with excretion rate dependent on urinary pH.
Half life
Approximately 3-6 hours.
Absorption
Rapid and complete absorption after intramuscular (IM) or subcutaneous (SC) administration.
Metabolism
Partially metabolized in the liver by oxidative deamination.
Side Effects
Contraindications
- •Hypersensitivity to ephedrine or any component of the formulation.
- •Narrow-angle glaucoma.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy.
- •Severe hypertension, severe coronary artery disease, or other cardiovascular diseases where increased blood pressure would be dangerous.
- •Thyrotoxicosis, pheochromocytoma.
Drug Interactions
MAOIs
Concurrent use can lead to severe hypertensive crisis.
Beta-blockers
May antagonize the bronchodilator and cardiac stimulant effects of ephedrine.
Alpha-blockers
May reduce the pressor effect of ephedrine.
Tricyclic antidepressants
May potentiate the pressor effects of ephedrine.
Halogenated anesthetics (e.g., halothane)
Increased risk of cardiac arrhythmias.
Storage
Store at controlled room temperature (20-25°C), protect from light. Do not freeze.
Overdose
Symptoms include tachycardia, hypertension, palpitations, headache, tremor, anxiety, seizures, and potentially coma. Management is symptomatic and supportive, which may include alpha-blockers for severe hypertension and benzodiazepines for seizures.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Excreted into breast milk; use with caution in breastfeeding mothers.
