Description
Overview of the medicine
Feglo-FZ is a combination medicine containing Fexofenadine (an antihistamine) and Montelukast (a leukotriene receptor antagonist). It is used to relieve symptoms of allergic rhinitis and chronic urticaria, and to help manage asthma in some cases.
Uses & Indications
Dosage
Adults
One tablet (Fexofenadine 48 mg + Montelukast 10 mg) once daily, preferably in the evening, with or without food.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential age-related decrease in renal or hepatic function.
Renal_impairment
Use with caution in patients with renal impairment. Lower starting dose or reduced frequency may be considered for Fexofenadine component.
How to Take
Take orally with a glass of water, with or without food. It is generally recommended to take the tablet in the evening.
Mechanism of Action
Fexofenadine selectively blocks peripheral H1-receptors, preventing histamine from causing allergy symptoms. Montelukast selectively blocks the action of leukotrienes at the CysLT1 receptor, thereby reducing inflammation, bronchoconstriction, and other allergic responses.
Pharmacokinetics
Onset
Fexofenadine: ~1 hour. Montelukast: ~24 hours for full effect in asthma.
Excretion
Fexofenadine is excreted mainly unchanged in feces (80%) and urine (11-12%). Montelukast is primarily excreted in feces via bile (approx. 86%).
Half life
Fexofenadine: 11-15 hours. Montelukast: 2.7-5.5 hours.
Absorption
Fexofenadine is rapidly absorbed, Tmax 2-3 hours. Montelukast is also rapidly absorbed, Tmax 2-4 hours.
Metabolism
Fexofenadine is minimally metabolized (approx. 5%). Montelukast is extensively metabolized by CYP3A4, 2C9, and 2C8.

Contraindications
- Hypersensitivity to Fexofenadine, Montelukast, or any excipients of the tablet.
Drug Interactions
Phenobarbital or Rifampin
May decrease plasma levels of Montelukast.
Erythromycin or Ketoconazole
May increase plasma levels of Fexofenadine.
Antacids (containing aluminum and magnesium)
May decrease the absorption of Fexofenadine. Separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
