Description
Overview of the medicine
Femoston 1/10 mg tablet is a type of hormone replacement therapy (HRT) used to relieve symptoms of menopause and prevent osteoporosis in postmenopausal women.
Uses & Indications
Dosage
Adults
One tablet daily. The pack contains 28 tablets; 14 white tablets (1 mg estradiol) for the first 14 days and 14 grey tablets (1 mg estradiol + 10 mg dydrogesterone) for the remaining 14 days. This regimen is sequential.
Elderly
No specific dosage adjustment is generally required for elderly women, but caution should be exercised and individual risk/benefit assessed.
Renal_impairment
No specific studies have been performed. Use with caution. Not recommended in severe renal impairment.
How to Take
Take one tablet orally with water, at approximately the same time each day, with or without food. Follow the sequential order of tablets in the pack.
Mechanism of Action
Estradiol replaces the estrogen deficiency in postmenopausal women, alleviating menopausal symptoms. Dydrogesterone is a progestogen that protects the endometrium from the proliferative effects of estrogen.
Pharmacokinetics
Onset
Symptomatic relief usually begins within a few weeks, with full effect observed after several months of continuous treatment.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates.
Half life
Estradiol: approximately 1-2 hours (active metabolites have longer half-lives). Dydrogesterone: approximately 5-7 hours.
Absorption
Estradiol is readily absorbed from the GI tract and undergoes significant first-pass metabolism. Dydrogesterone is rapidly absorbed.
Metabolism
Estradiol is primarily metabolized in the liver to estrone and estriol. Dydrogesterone is extensively metabolized in the liver to 20α-dihydrodydrogesterone (DHD), its main active metabolite.
Side Effects
Contraindications
- •Known, past, or suspected breast cancer
- •Known or suspected estrogen-dependent malignant tumours
- •Undiagnosed vaginal bleeding
- •Untreated endometrial hyperplasia
- •Previous idiopathic or current venous thromboembolism (DVT, PE)
- •Active or recent arterial thromboembolic disease (angina, MI, stroke)
- •Acute liver disease or history of liver disease where liver function tests have failed to return to normal
- •Porphyria
- •Hypersensitivity to the active substances or to any of the excipients.
Drug Interactions
Ritonavir, nelfinavir (HIV medications)
Known as strong enzyme inducers, they may affect estrogen and progestogen metabolism.
Antimicrobials (e.g., rifampicin, rifabutin)
May decrease estrogen and progestogen levels by inducing liver enzymes.
Herbal preparations containing St. John's Wort
May induce liver enzymes, potentially reducing the effects of HRT.
Anticonvulsants (e.g., phenobarbital, phenytoin, carbamazepine)
May decrease estrogen and progestogen levels by inducing liver enzymes, leading to reduced efficacy and potentially more bleeding.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose of Femoston is generally well tolerated and unlikely to cause serious effects. Symptoms may include nausea, vomiting, breast tenderness, dizziness, abdominal pain, drowsiness/fatigue, and withdrawal bleeding. Treatment is symptomatic.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. If pregnancy occurs during treatment, Femoston should be discontinued immediately.
