Description
Overview of the medicine
Feofer 100 mg Injection is an intravenous iron preparation used to treat iron deficiency anemia when oral iron is ineffective, not tolerated, or inappropriate for rapid iron repletion.
Uses & Indications
Dosage
Adults
Individualized based on total iron deficit. Common regimen: 100 mg to 1000 mg total dose given as single or multiple intravenous infusions. 100 mg can be given as a single dose based on patient's iron deficiency status.
Elderly
No specific dose adjustment required for elderly patients, but caution is advised due to potential comorbidities and concomitant medications.
Renal_impairment
No specific dose adjustment is typically required in patients with renal impairment, including those with non-dialysis dependent chronic kidney disease. Monitoring of iron parameters is crucial.
How to Take
Administer as an intravenous infusion, typically diluted in 0.9% sodium chloride solution. Administer slowly over a recommended period (e.g., 6 to 15 minutes for 100 mg). Do not administer as an intravenous push.
Mechanism of Action
The iron in Ferric Carboxymaltose is delivered to cells of the reticuloendothelial system, which then release iron. This iron is then incorporated into transferrin and subsequently transported to the bone marrow for hemoglobin synthesis and also stored as ferritin.
Pharmacokinetics
Onset
Rapid increase in serum iron levels; significant increase in hemoglobin usually seen after several weeks.
Excretion
Minimal urinary excretion of intact Ferric Carboxymaltose; iron is primarily excreted via the gastrointestinal tract and sloughing of cells.
Half life
Terminal half-life is approximately 7-12 hours.
Absorption
Intravenous administration directly bypasses absorption, with iron complex entering systemic circulation.
Metabolism
The iron complex is taken up by the reticuloendothelial system, where iron is released from the complex and incorporated into transferrin and ferritin.
Side Effects
Contraindications
- •Known hypersensitivity to Ferric Carboxymaltose or to any of its excipients.
- •Anemia not attributed to iron deficiency (e.g., hemolytic anemia).
- •Evidence of iron overload or disturbances in iron utilization.
- •First trimester of pregnancy.
Drug Interactions
ACE Inhibitors
No known specific clinically significant interactions.
Oral Iron Preparations
Parenteral iron preparations may reduce the absorption of concurrently administered oral iron preparations. Oral iron therapy should not be started for at least 5 days after the last injection.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Overdose may lead to iron overload, which can manifest as hemosiderosis. Treatment should involve monitoring of iron parameters and, if necessary, the use of iron chelating agents (e.g., deferoxamine).
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if the potential benefit outweighs the potential risk to the fetus, particularly in the second and third trimesters. Not recommended in the first trimester. Exercise caution when administered to a nursing woman.
