Description
Overview of the medicine
Feromat 200 mg Syrup is an iron supplement used to treat and prevent iron deficiency anemia. It provides ferrous fumarate, a readily absorbed form of iron, essential for hemoglobin formation and red blood cell production.
Uses & Indications
Dosage
Adults
Adults: 5-10 ml (200-400 mg Ferrous Fumarate, 65-130 mg elemental iron) daily or as directed by physician. May be taken in divided doses.
Elderly
Elderly: Same as adult dosage, but caution with gastrointestinal sensitivity.
Renal_impairment
Renal impairment: No specific dose adjustment generally needed, but use with caution in severe cases due to potential for iron accumulation. Consult physician.
How to Take
Take orally, preferably after meals or with food to minimize gastrointestinal upset. Do not lie down for at least 30 minutes after taking. Shake well before use. May be diluted with water or fruit juice (excluding milk or tea).
Mechanism of Action
Iron is an essential constituent of hemoglobin, myoglobin, and a number of enzymes. Ferrous Fumarate provides elemental iron, which is necessary for the synthesis of heme, a component of hemoglobin. It helps restore iron levels in the body, facilitating the formation of oxygen-carrying red blood cells.
Pharmacokinetics
Onset
Hematologic response usually begins within 7-10 days, with noticeable improvement in symptoms within a few weeks.
Excretion
The body has no physiological mechanism for actively excreting iron. Small amounts are lost through shedding of skin and mucosal cells, sweat, urine, and feces. Menstrual blood loss contributes significantly in women.
Half life
Not directly applicable for elemental iron as it is stored and utilized, but unabsorbed iron is excreted. Serum iron has a short half-life due to rapid uptake by cells.
Absorption
Iron is absorbed primarily in the duodenum and proximal jejunum. Ferrous iron is more readily absorbed than ferric iron. Absorption is enhanced by Vitamin C and reduced by antacids, tetracyclines, and food components like phytates and phosphates.
Metabolism
Iron is primarily stored in the body as ferritin in the liver, spleen, and bone marrow, and as hemosiderin. It is not metabolized in the traditional sense.
Side Effects
Contraindications
- Hypersensitivity to ferrous fumarate or any component of the syrup.
- Haemochromatosis and hemosiderosis (conditions of iron overload).
- Other non-iron deficiency anemias (e.g., hemolytic anemia, megaloblastic anemia unless iron deficiency is co-existent).
- Repeated blood transfusions.
Drug Interactions
Antacids
Antacids can decrease iron absorption. Take iron at least 2 hours before or 4 hours after antacids.
Methyldopa
Reduced absorption of methyldopa when taken with iron.
Levothyroxine
Iron can reduce the absorption of levothyroxine. Separate administration by at least 4 hours.
Penicillamine
Iron reduces the absorption of penicillamine. Administer at least 2 hours apart.
Tetracyclines and Quinolones
Iron can decrease the absorption of these antibiotics. Separate administration by at least 2-3 hours.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep tightly closed.
Overdose
Symptoms of acute iron overdose include severe gastrointestinal irritation, abdominal pain, nausea, vomiting (may be bloody), diarrhea, metabolic acidosis, shock, and liver damage. Seek immediate medical attention or contact a poison control center.
Pregnancy & Lactation
Iron supplementation is often essential and recommended during pregnancy and lactation to meet increased demands and prevent maternal and infant iron deficiency. Ferrous fumarate is generally considered safe. Always consult a healthcare provider.

YesClinical Trials
Ferrous fumarate is a well-established iron supplement with extensive clinical data supporting its efficacy and safety in treating iron deficiency anemia. Numerous studies have confirmed its role in increasing hemoglobin levels and improving iron stores.
Lab Monitoring
- Complete Blood Count (CBC) including Hemoglobin (Hb) and Hematocrit (Hct)
- Serum Ferritin levels
- Serum Iron and Total Iron Binding Capacity (TIBC)
Doctor Notes
- Emphasize the importance of patient compliance for effective treatment.
- Advise patients about potential gastrointestinal side effects and ways to manage them (e.g., taking with food, starting with lower dose).
- Counsel on dietary modifications to enhance iron absorption and avoid inhibitors.
- Monitor hemoglobin and ferritin levels periodically.
Patient Guidelines
- Take the medicine as prescribed by your doctor.
- Do not stop taking iron supplements prematurely, even if you feel better. Iron stores take time to replenish.
- Report any severe side effects to your doctor.
- Keep out of reach of children to prevent accidental poisoning.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and take your next dose at the regular time. Do not double the dose to catch up.
Driving Precautions
Feromat Syrup is not known to affect the ability to drive or operate machinery. If you experience dizziness or fatigue, avoid such activities.
Lifestyle Advice
- Consume a balanced diet rich in iron (e.g., red meat, fortified cereals, spinach).
- Include Vitamin C-rich foods (e.g., citrus fruits, bell peppers) to enhance iron absorption.
- Avoid taking iron supplements with tea, coffee, or milk.
