Description
Overview of the medicine
Feroplus TR 200 mg capsule is an iron supplement used to treat and prevent iron deficiency anemia. The 'TR' indicates Timed Release, which allows for a gradual absorption of iron, potentially reducing gastrointestinal side effects.
Uses & Indications
Dosage
Adults
Typically one capsule daily, or as directed by physician. May be increased to twice daily in severe cases.
Elderly
Same as adult dosage, but caution for underlying conditions.
Renal_impairment
Use with caution. Dosage adjustment may not be required unless severe anemia necessitating erythropoietin.
How to Take
Take orally with a glass of water. Can be taken with or without food. Taking with food may reduce gastrointestinal discomfort but may slightly impair absorption. Do not lie down for at least 10 minutes after taking.
Mechanism of Action
Ferrous fumarate provides exogenous iron, which is an essential component of hemoglobin, myoglobin, and various enzymes. Iron is crucial for oxygen transport, energy metabolism, and cellular growth. In iron deficiency, it helps restore depleted iron stores and improve erythropoiesis.
Pharmacokinetics
Onset
Hemoglobin increase typically seen after 2-4 weeks.
Excretion
Primarily through shedding of mucosal cells, bile, sweat, and urine (minimal).
Half life
Iron has no true 'half-life' in the pharmacological sense as it's recycled. The half-life of absorbed iron bound to transferrin is short (60-90 minutes).
Absorption
Absorbed primarily in the duodenum and upper jejunum. Timed-release formulation ensures gradual absorption.
Metabolism
Iron is not metabolized in the traditional sense; it's incorporated into hemoglobin and other proteins, or stored.
Side Effects
Contraindications
- •Hypersensitivity to ferrous fumarate or any component
- •Hemochromatosis and hemosiderosis (iron overload disorders)
- •Non-iron deficiency anemia (e.g., pernicious anemia, thalassemia)
- •Repeated blood transfusions
Drug Interactions
Antacids
May reduce iron absorption. Take iron at least 2 hours before or 4 hours after antacids.
Levothyroxine
Iron may reduce absorption of levothyroxine. Administer at least 4 hours apart.
Penicillamine
Reduced absorption of penicillamine.
Tetracyclines & Quinolones
Iron can chelate with these antibiotics, reducing their absorption. Take iron at least 2 hours before or 4-6 hours after these antibiotics.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of iron overdose include severe gastrointestinal distress (nausea, vomiting, diarrhea, abdominal pain), pallor, cyanosis, shock, and coma. In case of overdose, seek immediate medical attention. Treatment involves supportive care, gastric lavage, and potentially chelation therapy (e.g., deferoxamine).
Pregnancy & Lactation
Iron supplementation is often necessary and recommended during pregnancy and lactation to meet increased iron demands and prevent deficiency. Consult a doctor before use.
