Description
Overview of the medicine
Ferritin-TR is a sustained-release capsule containing Iron Polymaltose Complex and Folic Acid, used to treat and prevent various types of anemia, especially iron deficiency anemia and megaloblastic anemia, particularly during pregnancy and lactation.
Uses & Indications
Dosage
Adults
Typically, 1 capsule once daily, or as directed by the physician. Duration of treatment depends on the severity of the deficiency.
Elderly
No specific dosage adjustment required for elderly patients, but use with caution in those with pre-existing conditions.
Renal_impairment
Use with caution in patients with severe renal impairment. Dosage adjustment may be necessary based on physician's assessment.
How to Take
Take orally, preferably with or after meals to minimize gastrointestinal discomfort. Do not chew or crush the capsule; swallow whole.
Mechanism of Action
Iron Polymaltose Complex provides elemental iron, which is essential for the formation of hemoglobin and red blood cells. Folic acid is vital for DNA synthesis, cell division, and erythropoiesis (red blood cell production).
Pharmacokinetics
Onset
Improvements in hemoglobin levels typically observed within 2-4 weeks.
Excretion
Iron excretion is minimal, primarily through feces, urine, and sweat. Folic acid and its metabolites are excreted primarily via urine.
Half life
Elemental iron does not have a typical half-life as it is utilized or stored. Folic acid's half-life is approximately 3-4 hours.
Absorption
Iron from IPC is absorbed via an active transport mechanism, minimizing gastrointestinal irritation. Folic acid is readily absorbed from the GI tract. Timed-release formulation ensures prolonged absorption.
Metabolism
Iron is incorporated into hemoglobin or stored as ferritin/hemosiderin. Folic acid is metabolized in the liver to its active form, tetrahydrofolate.
Side Effects
Contraindications
- •Known hypersensitivity to any component
- •Iron overload conditions (e.g., hemochromatosis, hemosiderosis)
- •Non-iron deficiency anemias (e.g., hemolytic anemia, aplastic anemia) unless iron deficiency is also present
Drug Interactions
Antacids
Reduce iron absorption
Levothyroxine
Iron reduces absorption
Tetracyclines
Form insoluble complexes with iron
Bisphosphonates
Iron reduces absorption
Chloramphenicol
May delay iron response
Quinolone antibiotics
Iron reduces absorption
Storage
Store in a cool, dry place below 30°C, protected from light and moisture.
Overdose
Symptoms of iron overdose include nausea, vomiting, abdominal pain, diarrhea, and in severe cases, metabolic acidosis, shock, and organ damage. Folic acid overdose is rare and generally non-toxic. In case of suspected overdose, seek immediate medical attention. Treatment may involve gastric lavage and chelation therapy.
Pregnancy & Lactation
Ferritin-TR is generally safe and often recommended during pregnancy and lactation for the prevention and treatment of iron and folic acid deficiency anemia. Consult a physician.
