Overview of the medicine
Fezin-CI 50 mg Capsule is an antidepressant medicine containing Citalopram, used to treat depression, panic disorder, and obsessive-compulsive disorder (OCD).
Uses & Indications
Dosage
Adults
Initial dose 20 mg once daily, may be increased to 40 mg or 60 mg daily based on response and tolerability. Max 60 mg/day for depression; 40 mg/day for panic disorder.
Elderly
Maximum recommended dose is 20 mg/day due to reduced metabolism and increased risk of side effects.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl <20 mL/min).
How to Take
Fezin-CI capsule should be taken orally once daily, with or without food, at any time of day but preferably at the same time each day.
Mechanism of Action
Citalopram selectively inhibits the reuptake of serotonin (5-HT) in the brain, leading to an increased concentration of serotonin in the synaptic cleft, thereby enhancing serotonergic neurotransmission.

Onset
Therapeutic effects may take 2-4 weeks to become apparent.
Excretion
Primarily via urine (approx. 10%) and feces (approx. 90%).
Half life
Approximately 35 hours (range 20-65 hours).
Absorption
Well absorbed orally, peak plasma concentrations typically within 2-4 hours.
Metabolism
Extensively metabolized in the liver by CYP2C19, CYP3A4, and CYP2D6 to inactive or weakly active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to citalopram or any excipients
- •Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of stopping an MAOI
- •Concomitant use with pimozide
- •Congenital long QT syndrome or known acquired QT prolongation
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
Pimozide
Increased QT prolongation.
Tramadol
Increased risk of serotonin syndrome.
Triptans
Increased risk of serotonin syndrome.
Warfarin
Possible alteration of anticoagulant effect; monitor INR.
Linezolid
Increased risk of serotonin syndrome.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
CYP2C19 inhibitors (e.g., omeprazole)
Increased citalopram levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, coma, seizures, tachycardia, ECG changes (QT prolongation), nausea, vomiting, tremor, dizziness. Treatment is supportive; secure airway, monitor cardiac rhythm and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Citalopram is excreted in human milk; caution when administered to a nursing woman.
