Description
Overview of the medicine
Fiasp Flextouch is an ultra-rapid-acting insulin analog containing insulin aspart, used to improve glycemic control in adults and children with diabetes mellitus. It is designed to act faster than traditional rapid-acting insulins.
Uses & Indications
Dosage
Adults
Individualized dosage based on patient's metabolic needs, blood glucose monitoring, and glycemic control goal. Administer immediately before a meal or within 20 minutes after starting a meal.
Elderly
Start with a lower dose and adjust carefully due to potential for renal impairment and increased sensitivity to insulin.
Renal_impairment
Dosage reduction may be necessary; monitor glucose levels closely and adjust accordingly.
How to Take
Administer by subcutaneous injection into the abdomen, thigh, upper arm, or buttock. Rotate injection sites within the same region to reduce the risk of lipodystrophy. Administer immediately before a meal or within 20 minutes after starting a meal.
Mechanism of Action
Insulin aspart lowers blood glucose by stimulating peripheral glucose uptake by skeletal muscle and fat, and by inhibiting hepatic glucose production. It also inhibits lipolysis and proteolysis, and enhances protein synthesis.
Pharmacokinetics
Onset
Approximately 2-4 minutes (faster than conventional insulin aspart).
Excretion
Primarily renal excretion of metabolites.
Half life
Approximately 8-9 minutes (after subcutaneous injection).
Absorption
Rapidly absorbed following subcutaneous injection, with a faster initial absorption rate due to the addition of niacinamide.
Metabolism
Degraded by insulin-degrading enzymes (insulinase) mainly in the liver and kidneys.
Side Effects
Contraindications
- Hypersensitivity to insulin aspart or any excipients
- During episodes of hypoglycemia
Drug Interactions
Alcohol
May potentiate the hypoglycemic effect of insulin.
Beta-blockers
May mask symptoms of hypoglycemia, requiring closer glucose monitoring.
Thiazolidinediones (TZDs)
Increased risk of fluid retention and heart failure when used with insulin.
Corticosteroids, Diuretics, Sympathomimetics
May decrease the glucose-lowering effect of insulin.
Storage
Unopened pens should be stored in a refrigerator (2°C to 8°C). Do not freeze. Opened or in-use pens can be stored at room temperature (below 30°C) for up to 28 days. Do not refrigerate opened pens.
Overdose
Overdose of insulin can lead to severe hypoglycemia. Management involves oral glucose for mild cases, or intravenous glucose/glucagon injection for severe cases, followed by monitoring.
Pregnancy & Lactation
Insulin is the preferred treatment for diabetes in pregnancy. Fiasp can be used, but close monitoring of glucose control is essential. Insulin aspart is considered compatible with breastfeeding.

Lab Monitoring
- Frequent monitoring of blood glucose levels (fasting, pre-meal, post-meal, bedtime)
- Glycated hemoglobin (HbA1c) periodically
- Kidney and liver function periodically
Doctor Notes
- Emphasize thorough patient education on hypoglycemia recognition and management, including carrying emergency glucose.
- Stress the importance of adherence to blood glucose monitoring and timely dosage adjustments based on patient's lifestyle and glycemic targets.
- Advise careful titration and close monitoring in elderly patients and those with renal or hepatic impairment due to increased sensitivity or altered insulin clearance.
Patient Guidelines
- Always check the insulin label and strength before each injection to avoid medication errors.
- Understand the symptoms of hypoglycemia and how to treat them effectively.
- Rotate injection sites to prevent lipodystrophy and ensure consistent absorption.
- Never share your insulin pen or needles with others.
Missed Dose Advice
If a dose is missed, monitor blood glucose levels and resume the normal dosing schedule with the next meal. Do not take a double dose to compensate for a missed one.
Driving Precautions
Patients with diabetes should be aware of the symptoms of hypoglycemia. Hypoglycemia may impair the ability to concentrate and react, potentially affecting the ability to drive or operate machinery. Check blood glucose before driving if necessary.
Lifestyle Advice
- Follow a balanced diet and maintain consistent meal times as advised by your healthcare provider.
- Engage in regular physical activity, adjusting insulin doses as needed to prevent hypoglycemia.
- Regularly monitor blood sugar levels and record results to help adjust your insulin dosage.
- Wear medical identification indicating you have diabetes.
