Description
Overview of the medicine
Filgrastim is a recombinant human granulocyte colony-stimulating factor (G-CSF) used to stimulate the production of neutrophils (a type of white blood cell) in the bone marrow. It is used to prevent or treat neutropenia, a condition characterized by a low count of neutrophils, which can increase the risk of infection.
Uses & Indications
Dosage
Adults
Typically 5 mcg/kg/day, administered as a single daily subcutaneous injection or intravenous infusion. Dosage may be adjusted based on neutrophil response and clinical need.
Elderly
No specific dosage adjustment required based on age alone; monitor renal function and adjust as necessary.
Renal_impairment
No specific dosage adjustment recommended for patients with renal impairment, but caution is advised and monitoring is important.
How to Take
Administer by subcutaneous injection or intravenous infusion. Subcutaneous administration is preferred for most indications. Intravenous infusion can be administered over 15-30 minutes.
Mechanism of Action
Filgrastim binds to specific G-CSF receptors on hematopoietic stem cells and progenitor cells, stimulating their proliferation, differentiation, and functional activation. This leads to an increase in neutrophil production and release from the bone marrow.
Pharmacokinetics
Onset
Increase in neutrophil count observed within 24 hours.
Excretion
Mainly renal excretion of intact drug and metabolites.
Half life
Approximately 3.5 hours after subcutaneous administration; slightly longer in patients with severe neutropenia.
Absorption
Rapidly absorbed after subcutaneous administration, with peak serum concentrations achieved in 3-5 hours.
Metabolism
Primarily catabolized by neutrophils and renal excretion.
Side Effects
Contraindications
- History of serious allergic reactions to filgrastim or pegfilgrastim.
- Patients with severe chronic neutropenia with abnormal cytogenetics.
Drug Interactions
Lithium
May potentiate the myeloproliferative effects of filgrastim.
Myelosuppressive chemotherapy and radiation therapy
Filgrastim should not be administered in the 24 hours before and 24 hours after administration of cytotoxic chemotherapy.
Storage
Store refrigerated at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light. Do not shake.
Overdose
Effects of filgrastim overdose have not been established. Excessive doses might result in an exaggerated pharmacological effect, such as an increase in white blood cell counts. Discontinuation of therapy generally leads to a return to baseline counts.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is unknown whether filgrastim is excreted in human milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Complete Blood Count (CBC) with differential, especially neutrophil count, regularly during treatment.
- Monitor splenic size regularly, especially in patients with chronic severe neutropenia.
Doctor Notes
- Ensure absolute neutrophil count (ANC) is monitored regularly.
- Educate patients on proper injection technique if self-administering.
- Advise patients to report any severe abdominal or shoulder pain immediately, as it may indicate splenic rupture.
- Avoid concomitant use with other myelostimulatory agents unless specifically indicated.
Patient Guidelines
- Do not shake the syringe vigorously.
- Store in the refrigerator, do not freeze.
- Do not administer if the solution is discolored or cloudy.
- Report any signs of allergic reaction or pain in the upper left abdomen immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose.
Driving Precautions
Filgrastim is not known to impair the ability to drive or operate machinery. However, if you experience dizziness or fatigue, avoid such activities.
Lifestyle Advice
- Maintain good hygiene to reduce infection risk.
- Avoid contact with people who are sick.
