Description
Overview of the medicine
Fixal contains Fexofenadine Hydrochloride, a non-sedating antihistamine used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps alleviate sneezing, runny nose, itchy nose/palate/throat, itchy/watery/red eyes, and skin itching and rash.
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis: 60 mg twice daily or 120 mg once daily or 180 mg once daily. For chronic idiopathic urticaria: 60 mg twice daily or 180 mg once daily.
Elderly
No dosage adjustment is generally needed based on age alone, but renal function should be considered.
Renal_impairment
For patients with impaired renal function (creatinine clearance less than 80 mL/min): Start with 60 mg once daily for both indications.
How to Take
Take the tablet orally with water. It is recommended to take it before a meal, as absorption may be reduced by high-fat meals and fruit juices. Do not take with fruit juices like grapefruit, orange, or apple juice.
Mechanism of Action
Fexofenadine is a selective peripheral H1-blocker. It binds to histamine H1 receptors, preventing histamine from exerting its effects, thereby reducing allergic symptoms without causing significant central nervous system depression.
Pharmacokinetics
Onset
Onset of action is usually within 1 hour.
Excretion
Primarily excreted unchanged in feces (about 80%) and urine (about 11%).
Half life
The elimination half-life is approximately 14.4 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached in 1-3 hours.
Metabolism
Minimally metabolized; approximately 5% undergoes hepatic metabolism.
Side Effects
Contraindications
- Known hypersensitivity to Fexofenadine Hydrochloride or any of the excipients.
Drug Interactions
Erythromycin or Ketoconazole
Can increase Fexofenadine plasma concentrations (though clinical significance is minimal).
Antacids (aluminum and magnesium hydroxide containing)
Can decrease absorption of Fexofenadine. Separate administration by at least 2 hours.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, patients have reported dizziness, drowsiness, and dry mouth. Management should be symptomatic and supportive, including gastric lavage and observation.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Fexofenadine is excreted in human milk; however, other antihistamines are. Use with caution in nursing mothers.
