Description
Overview of the medicine
Fizycal-M Tablet is a combination of calcium carbonate and Vitamin D3, used to supplement calcium and Vitamin D in the body, which are essential for bone health and various metabolic functions.
Uses & Indications
Dosage
Adults
1-2 tablets daily, preferably with meals or as directed by physician.
Elderly
Same as adult dosage, or as directed by physician, with caution for renal function.
Renal_impairment
Use with caution; dose adjustment may be required based on calcium and Vitamin D levels. Consult a physician.
How to Take
Take orally, preferably with a meal to enhance absorption. Do not crush or chew if it is not a chewable tablet.
Mechanism of Action
Calcium carbonate provides elemental calcium essential for bone mineralization, muscle contraction, and nerve function. Vitamin D3 facilitates the absorption of calcium from the gut and its deposition into bones.
Pharmacokinetics
Onset
Nutritional effects are gradual, over days to weeks.
Excretion
Calcium: Primarily fecal, some renal. Vitamin D3: Metabolites excreted mainly in bile and feces, small amount in urine.
Half life
Calcium: Varies. Vitamin D3: 24-48 hours (parent compound); active metabolites have longer half-lives (e.g., calcifediol ~16 days).
Absorption
Calcium carbonate is absorbed in the small intestine, with absorption enhanced by Vitamin D3. Vitamin D3 is absorbed from the small intestine, metabolized in the liver and kidneys.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol, the active form).
Side Effects
Contraindications
- •Hypercalcemia (high calcium levels in blood).
- •Hypercalciuria (high calcium levels in urine).
- •Severe renal impairment.
- •Hypersensitivity to calcium carbonate or Vitamin D3.
- •Nephrolithiasis (kidney stones).
Drug Interactions
Digoxin
May increase risk of cardiac arrhythmias if hypercalcemia is present.
Thyroid hormones
May reduce absorption of levothyroxine; take calcium 4 hours apart.
Thiazide diuretics
May increase risk of hypercalcemia.
Tetracyclines & Fluoroquinolones
May reduce absorption of antibiotics; take calcium 2-4 hours apart.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, bone pain, kidney stones). Treatment involves discontinuing the supplement, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended doses for calcium and Vitamin D supplementation. Consult a physician before use.
