Overview of the medicine
Flexi 100 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation associated with various musculoskeletal conditions, such as osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. It works by reducing the production of certain natural substances in the body that cause inflammation.
Uses & Indications
Dosage
Adults
The recommended dose is 100 mg twice daily, one tablet in the morning and one in the evening. It should be taken with or after food.
Elderly
No specific dose adjustment is usually required, but caution should be exercised. The lowest effective dose should be used for the shortest possible duration.
Renal_impairment
In mild to moderate renal impairment, no dose adjustment is usually necessary. However, caution is advised, and regular monitoring of renal function is recommended. Contraindicated in severe renal impairment.
How to Take
Take the tablet whole with a glass of water, with or immediately after food. Do not chew or crush the tablet.
Mechanism of Action
Flexi (Aceclofenac) primarily acts by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which are responsible for the synthesis of prostaglandins. Prostaglandins are inflammatory mediators that cause pain, swelling, and fever. By reducing prostaglandin production, Aceclofenac exerts its anti-inflammatory, analgesic, and antipyretic effects.

Onset
Within 30-60 minutes for analgesic effect.
Excretion
Excreted mainly via urine (approximately two-thirds) and faeces (approximately one-third) as metabolites. Only a small fraction is excreted as unchanged drug.
Half life
Approximately 4-4.3 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are reached approximately 1.25 to 3 hours after administration.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 (CYP2C9) to 4'-hydroxyaceclofenac, its main metabolite.
Side Effects
Contraindications
- Hypersensitivity to Aceclofenac or any other NSAID (e.g., aspirin, ibuprofen)
- History of gastrointestinal bleeding or perforation related to previous NSAID therapy
- Active or recurrent peptic ulcer/haemorrhage
- Severe heart failure, hepatic failure, or renal failure
- Last trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased plasma levels of methotrexate, potentially leading to toxicity.
Other NSAIDs
Increased risk of adverse effects, concurrent use not recommended.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced efficacy of these drugs and increased risk of renal impairment.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhoea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting and convulsions. Management typically involves symptomatic and supportive treatment. Gastric lavage and activated charcoal may be considered if ingestion was recent. Monitor renal function and blood pressure.
Pregnancy & Lactation
Flexi 100 mg Tablet should be avoided during the third trimester of pregnancy due to potential risks to the fetus and complications during delivery. During the first two trimesters, it should only be used if the potential benefit outweighs the risk and under strict medical supervision. It is not recommended during breastfeeding as Aceclofenac may pass into breast milk.
