Overview of the medicine
Flucid (Fluconazole) is an antifungal medication used to treat a variety of fungal and yeast infections. It belongs to the triazole class of antifungal agents.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: A single 150 mg oral dose. For oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days. For systemic candidiasis: 400 mg on day 1, then 200-400 mg once daily. Dosage varies significantly by indication; consult a physician.
Elderly
No dose adjustment is typically needed unless there is evidence of renal impairment. Monitor renal function.
Renal_impairment
For patients with creatinine clearance less than 50 mL/min, the dose should be reduced by 50% after a normal initial loading dose. For hemodialysis patients, a single dose should be given after each dialysis session.
How to Take
Flucid capsules can be taken orally, with or without food. Swallow the capsule whole with a glass of water. Do not chew or crush.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14α-demethylation of lanosterol, which is an essential step in fungal ergosterol biosynthesis. This inhibition leads to the accumulation of 14α-methyl sterols and a decrease in ergosterol, thereby impairing fungal cell membrane function.

Onset
Clinical improvement typically seen within a few days, depending on the type and severity of infection.
Excretion
Primarily renal; approximately 80% of the dose is excreted unchanged in the urine. Renal clearance is proportional to creatinine clearance.
Half life
Approximately 30 hours, allowing for once-daily dosing for many indications.
Absorption
Well absorbed orally, with bioavailability greater than 90%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Minimally metabolized; approximately 11% of the dose is metabolized to less active metabolites. It is a potent inhibitor of CYP2C9 and a moderate inhibitor of CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to fluconazole or any of its excipients.
- Co-administration with drugs known to prolong the QT interval and which are metabolized by the cytochrome P450 (CYP) 3A4 enzyme, such as terfenadine and cisapride.
Drug Interactions
Warfarin
Increased prothrombin time; monitor INR closely.
Phenytoin
Increased phenytoin levels; monitor phenytoin concentrations.
Rifampicin
Decreased fluconazole levels; consider increasing fluconazole dose.
Cyclosporine
Increased cyclosporine levels; monitor cyclosporine concentrations.
Hydrochlorothiazide
Increased plasma concentrations of fluconazole.
Statins (e.g., atorvastatin)
Increased statin levels, leading to increased risk of myopathy/rhabdomyolysis.
Benzodiazepines (e.g., midazolam)
Increased benzodiazepine levels; monitor for sedation.
Oral hypoglycemics (e.g., sulfonylureas)
Increased plasma concentration of sulfonylureas; monitor blood glucose.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hallucinations and paranoid behavior. Treatment should be symptomatic and supportive. Gastric lavage may be considered. Hemodialysis significantly reduces plasma levels of fluconazole.
Pregnancy & Lactation
Pregnancy: Category D for prolonged or high-dose use; Category C for single-dose use in vaginal candidiasis. Should only be used if the potential benefit justifies the potential risk to the fetus. Lactation: Fluconazole is excreted in breast milk. Caution should be exercised when administered to a nursing woman.
