Overview of the medicine
Flucloxacillin is an antibiotic used to treat a wide range of bacterial infections, particularly those caused by penicillinase-producing Staphylococci and Streptococci. It is effective against skin and soft tissue infections, bone and joint infections, respiratory tract infections, and other systemic infections.
Uses & Indications
Dosage
Adults
Oral: 250-500 mg every 6 hours, usually 30-60 minutes before meals. For severe infections, up to 1 g every 6 hours.
Elderly
Similar to adult dosage, but dose adjustment may be necessary in patients with significant renal impairment.
Renal_impairment
For creatinine clearance <10 mL/min, the dose should be reduced or the dosage interval extended (e.g., 250 mg every 8-12 hours).
How to Take
Take orally, preferably 30-60 minutes before meals, with a glass of water. Do not chew or crush the capsule.
Mechanism of Action
Flucloxacillin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Its beta-lactam structure is protected against hydrolysis by beta-lactamase enzymes, making it effective against beta-lactamase producing bacteria.

Onset
Rapid, with therapeutic concentrations achieved soon after administration.
Excretion
Primarily excreted by the kidneys, with 60-70% of the dose eliminated unchanged in the urine within 6 hours. Also undergoes biliary excretion.
Half life
Approximately 0.5 to 1.5 hours in adults with normal renal function.
Absorption
Well absorbed orally, especially when taken on an empty stomach. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites. About 10-20% of the drug is metabolized.
Side Effects
Contraindications
- Known hypersensitivity to penicillin or other beta-lactam antibiotics
- History of flucloxacillin-associated jaundice or hepatic dysfunction
Drug Interactions
Warfarin
Flucloxacillin may increase the anticoagulant effect of warfarin. Close monitoring of INR is required.
Methotrexate
Flucloxacillin may decrease the renal excretion of methotrexate, increasing its toxicity.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise additional contraceptive methods.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include gastrointestinal disturbances like nausea, vomiting, and diarrhea, as well as electrolyte imbalance. Treatment is symptomatic and supportive. Discontinue the drug.
Pregnancy & Lactation
Pregnancy Category B. Flucloxacillin crosses the placenta but is not associated with an increased risk of malformations. Use with caution. Generally considered safe during lactation, but small amounts are excreted in breast milk; monitor infants for gastrointestinal disturbances or candidiasis.
