Description
Overview of the medicine
Flumetol 25 mcg Inhaler contains Salmeterol Xinafoate, a long-acting beta2-agonist used for the maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It helps to open up the airways in the lungs, making breathing easier.
Uses & Indications
Dosage
Adults
Two inhalations (25 mcg/puff) twice daily, approximately 12 hours apart.
Elderly
No dosage adjustment required for elderly patients. Monitor for adverse effects.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
For oral inhalation only. Shake the inhaler well before each use. Follow proper inhaler technique as instructed by your doctor or pharmacist.
Mechanism of Action
Salmeterol selectively stimulates beta2-adrenergic receptors in the bronchial smooth muscle. This stimulation leads to the relaxation of the smooth muscle, causing bronchodilation and improved airflow in the lungs.
Pharmacokinetics
Onset
Bronchodilation typically occurs within 10-20 minutes after inhalation.
Excretion
Approximately 60% of the dose is excreted in feces and 25% in urine over 72 hours.
Half life
Plasma elimination half-life is approximately 5.5 hours.
Absorption
Rapidly absorbed from the lungs; systemic absorption is low.
Metabolism
Extensively metabolized by hydroxylation via cytochrome P450 3A4 (CYP3A4).
Side Effects
Contraindications
- Primary treatment of acute episodes of bronchospasm (it is not a rescue inhaler).
- Hypersensitivity to salmeterol or any component of the formulation.
Drug Interactions
MAOIs/TCAs
Potentiate the effect of salmeterol on the cardiovascular system.
Beta-blockers
May block the pulmonary effect of beta-agonists and produce severe bronchospasm in COPD and asthmatic patients.
CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir)
Can increase systemic exposure to salmeterol, increasing the risk of cardiovascular adverse effects.
Storage
Store below 30°C. Protect from direct sunlight and heat. Do not puncture or incinerate the canister, even when empty.
Overdose
Symptoms of overdose include tremor, palpitations, headache, tachycardia, and hypokalemia. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Use with caution in nursing mothers.

YesClinical Trials
Salmeterol has been extensively studied in clinical trials demonstrating its efficacy and safety in the maintenance treatment of asthma and COPD. Key trials have evaluated its bronchodilatory effects and impact on lung function and exacerbation rates.
Lab Monitoring
- Monitor serum potassium levels, especially in patients receiving concomitant therapy that can induce hypokalemia.
- Monitor for signs of cardiovascular effects (e.g., heart rate, blood pressure).
Doctor Notes
- Emphasize that Salmeterol is for maintenance treatment only and not for acute bronchospasm.
- For asthma patients, Salmeterol must always be prescribed with an inhaled corticosteroid.
- Educate patients on proper inhaler technique and the importance of regular follow-up.
Patient Guidelines
- Do not use Flumetol for acute asthma attacks or acute exacerbations of COPD. Always carry a rescue inhaler for immediate relief.
- Do not exceed the prescribed dose. Overuse can lead to serious cardiovascular effects.
- Ensure proper inhaler technique to maximize drug delivery to the lungs.
- If you have asthma, Flumetol must be used with an inhaled corticosteroid.
Missed Dose Advice
If a dose is missed, take the next dose at the regularly scheduled time. Do not take an extra dose to make up for a missed one.
Driving Precautions
Flumetol is not expected to impair the ability to drive or operate machinery. However, if side effects like tremor or dizziness occur, avoid such activities.
Lifestyle Advice
- Avoid smoking and exposure to respiratory irritants that can worsen asthma or COPD.
- Maintain a healthy lifestyle, including regular exercise (as tolerated) and a balanced diet.
