Overview of the medicine
Fluver (Flunarizine) is a calcium entry blocker indicated for the prophylaxis of migraine and symptomatic treatment of vestibular vertigo. It helps reduce the frequency and severity of migraine attacks and alleviates symptoms of dizziness.
Uses & Indications
Dosage
Adults
Migraine prophylaxis: Initially 5 mg once daily at bedtime for patients under 65 years. If insufficient response, may increase to 10 mg once daily. For vertigo: 10 mg once daily at bedtime for patients under 65 years for symptomatic control, usually not exceeding 2 months.
Elderly
For both indications, 5 mg once daily at bedtime, due to increased risk of side effects.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised. Consult a doctor.
How to Take
Take orally, preferably at night, with a glass of water. Can be taken with or without food.
Mechanism of Action
Flunarizine selectively inhibits the influx of calcium into vascular smooth muscle cells and brain cells, especially during ischemic events or migraine attacks. It stabilizes cellular membranes, has anti-serotonergic and antihistaminic properties, and reduces vestibular excitability. This action helps to prevent cerebral vasospasm and excessive neuronal firing associated with migraine and vertigo.

Onset
Therapeutic effects for migraine prophylaxis may take several weeks (e.g., 2-3 months) to become apparent.
Excretion
Primarily excreted via the faeces (bile), with a small amount excreted in the urine.
Half life
Long elimination half-life, ranging from 18-23 days, allowing for once-daily dosing.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 2-4 hours after oral administration.
Metabolism
Extensively metabolized in the liver, primarily via the CYP2D6 enzyme system.
Side Effects
Contraindications
- •Known hypersensitivity to flunarizine or any excipients.
- •History of depressive illness or pre-existing symptoms of Parkinson's disease or other extrapyramidal disorders.
- •Acute porphyria.
Drug Interactions
Beta-blockers
May increase the risk of adverse effects like hypotension or bradycardia.
Oral Contraceptives
No significant interaction observed, but some reports suggest potential for galactorrhea in combination.
Phenytoin, Carbamazepine
May reduce the plasma levels of flunarizine.
CNS Depressants (e.g., alcohol, hypnotics, anxiolytics)
May enhance sedative effects.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include sedation, bradycardia, hypotension, drowsiness, and disorientation. Management involves gastric lavage, activated charcoal, and symptomatic and supportive care. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Flunarizine is excreted in breast milk; therefore, avoid use during breastfeeding or discontinue breastfeeding while on treatment.
