Description
Overview of the medicine
Folvit TR contains Folic Acid, a B vitamin essential for DNA synthesis, cell division, and red blood cell formation. The 'TR' (Time Release) formulation ensures a gradual release of the active ingredient, maintaining stable blood levels over an extended period. It is primarily used to treat and prevent folic acid deficiency and associated conditions like megaloblastic anemia.
Uses & Indications
Dosage
Adults
For deficiency: 5 mg daily. For prevention of neural tube defects: 0.4-5 mg daily (depending on risk factors). Dosage should be adjusted based on individual needs and physician's advice.
Elderly
No specific dosage adjustment required, but monitor for concomitant conditions and medications.
Renal_impairment
No specific dosage adjustment needed as folic acid is generally safe and water-soluble. However, in severe renal impairment, dose may be reviewed.
How to Take
Oral administration. Tablets should be swallowed whole with water, preferably at the same time each day. Do not chew or crush sustained-release tablets.
Mechanism of Action
Folic acid is reduced in the body to tetrahydrofolate, which is a coenzyme for various metabolic processes, including the synthesis of purine and pyrimidine nucleotides and the interconversion of amino acids. It is crucial for DNA and RNA synthesis, and thus for cell division and growth, especially rapid cell proliferation like in erythropoiesis (red blood cell production).
Pharmacokinetics
Onset
Hematological response usually observed within 1-2 weeks.
Excretion
Primarily excreted in the urine, with some biliary excretion.
Half life
Approximately 30-60 minutes for unconjugated folic acid, but the duration of effect for the TR formulation is longer due to sustained release.
Absorption
Well absorbed from the gastrointestinal tract, primarily from the duodenum and jejunum. Time-release formulation provides extended absorption.
Metabolism
Metabolized in the liver to tetrahydrofolate and other active forms.
Side Effects
Contraindications
- •Hypersensitivity to folic acid or any component of the formulation
- •Untreated pernicious anemia or other vitamin B12 deficiencies (folic acid can mask B12 deficiency symptoms, leading to neurological damage)
Drug Interactions
Phenytoin
Folic acid may reduce phenytoin levels, increasing seizure risk. Phenytoin may reduce folic acid absorption.
Methotrexate
Folic acid may reduce the efficacy of methotrexate (a folate antagonist), used in cancer therapy and rheumatoid arthritis. Use with caution or as part of a rescue protocol.
Trimethoprim
A folate antagonist, may increase risk of megaloblastic anemia with concomitant use.
Sulfasalazine
May inhibit folic acid absorption.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children.
Overdose
Folic acid has a very low toxicity profile. Acute overdose is rare and generally not associated with severe adverse effects. Very high doses may rarely cause gastrointestinal upset or sleep disturbances. Management is supportive.
Pregnancy & Lactation
Folic acid is essential during pregnancy for the prevention of neural tube defects and is generally considered safe. It is also safe during lactation, as it is naturally present in breast milk.
