Description
Overview of the medicine
Folzin (Folic Acid) is a B vitamin essential for various metabolic processes, including DNA synthesis and cell division. It is primarily used to treat and prevent megaloblastic anemia caused by folate deficiency and is crucial for pregnant women to prevent neural tube defects in the fetus.
Uses & Indications
Dosage
Adults
For megaloblastic anemia: 5 mg daily for 4 months, which may be continued if necessary. For prevention of neural tube defects: 0.4 mg to 5 mg daily, starting before conception and continuing through the first trimester.
Elderly
No specific dose adjustment is usually necessary. Use adult dosage.
Renal_impairment
No specific dose adjustment is usually necessary. Folic acid is largely water-soluble and excreted unchanged or as metabolites in urine.
How to Take
Take orally with or without food, usually once daily. It is generally recommended to take it at the same time each day.
Mechanism of Action
Folic acid is metabolically inactive but is converted to tetrahydrofolate (THF) and other active folate derivatives in the body. THF is a coenzyme for single-carbon transfers in the synthesis of purine and pyrimidine nucleotides, which are essential for DNA and RNA synthesis. It is also involved in the metabolism of certain amino acids and the maturation of red blood cells.
Pharmacokinetics
Onset
Therapeutic effects on hematopoiesis may be seen within days.
Excretion
Primarily excreted in the urine, with some biliary excretion.
Half life
Approximately 4-5 hours for circulating folates.
Absorption
Readily absorbed from the gastrointestinal tract, primarily from the proximal small intestine. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Metabolized in the liver to tetrahydrofolate and other active forms.
Side Effects
Contraindications
- Known hypersensitivity to Folic Acid or any component of the formulation.
- Untreated pernicious anemia or other vitamin B12 deficiencies, as folic acid can mask the hematological symptoms while neurological damage progresses.
Drug Interactions
Methotrexate
Folic acid can reduce the effectiveness of methotrexate, an antifolate drug. Administer with caution and under medical supervision.
Sulfasalazine
May impair the absorption of folic acid.
Trimethoprim, Pyrimethamine
These antifolate drugs may interfere with folic acid metabolism, potentially reducing its effectiveness.
Antiepileptic drugs (e.g., Phenytoin, Phenobarbital, Primidone)
Folic acid may decrease the plasma concentrations of antiepileptic drugs, potentially leading to increased seizure frequency. Close monitoring is recommended.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Folic acid is generally considered non-toxic even in high doses. However, very high doses (e.g., >15 mg/day) may rarely cause gastrointestinal disturbances, sleep disturbances, or irritability. Management is typically supportive, as no specific antidote is available.
Pregnancy & Lactation
Folic acid is highly recommended before and during pregnancy to prevent neural tube defects. It is considered safe and essential during breastfeeding as well, providing necessary nutrients to the infant via breast milk.

WHO Essential Medicine
YesClinical Trials
Folic acid has been extensively studied in numerous clinical trials, particularly regarding its efficacy in preventing neural tube defects and treating megaloblastic anemia. Most studies have confirmed its safety and effectiveness.
Lab Monitoring
- Complete Blood Count (CBC) including hemoglobin and red blood cell indices to assess anemia.
- Serum folate levels to confirm deficiency and monitor treatment response.
- Serum Vitamin B12 levels should be checked, especially before starting folic acid, to rule out pernicious anemia.
Doctor Notes
- Emphasize the importance of vitamin B12 assessment before initiating folic acid therapy, especially in the elderly or those with risk factors for B12 deficiency.
- Advise pregnant patients on the critical timing of folic acid supplementation for neural tube defect prevention.
- Counsel patients on dietary sources of folate to complement supplementation.
Patient Guidelines
- Take Folzin exactly as prescribed by your doctor.
- Do not stop taking it without consulting your doctor, even if you feel better.
- Inform your doctor about all other medications and supplements you are taking.
- If you are pregnant or planning to become pregnant, discuss the importance of folic acid supplementation with your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Folzin (Folic Acid) is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- Maintain a balanced diet rich in folate (e.g., leafy green vegetables, citrus fruits, beans).
- Limit alcohol intake, as it can interfere with folic acid absorption.
- Regular health check-ups are important, especially if you have chronic conditions.
