Description
Overview of the medicine
Fomox XG eye drops combine Moxifloxacin, a broad-spectrum fluoroquinolone antibiotic, and Ketorolac, a non-steroidal anti-inflammatory drug (NSAID). It is used to treat bacterial conjunctivitis and to reduce ocular inflammation and pain, especially after ocular surgery.
Uses & Indications
Dosage
Adults
Typically, 1 drop in the affected eye(s) 3 times daily for 7-14 days, or as directed by a physician. For post-operative use, dosage might vary.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment required for renal impairment as systemic absorption is minimal.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch. Instill one drop into the pouch and close the eye gently. Avoid touching the dropper tip to any surface. If using other eye drops, wait at least 5 minutes between applications.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death. Ketorolac inhibits prostaglandin synthesis by blocking cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing inflammation, pain, and itching.
Pharmacokinetics
Onset
Antibacterial effects usually within hours, anti-inflammatory effects within minutes to hours.
Excretion
Primarily local elimination; minimal systemic excretion via urine/feces for absorbed amounts.
Half life
Not clinically significant due to minimal systemic absorption; local half-life not well-defined for topical ophthalmic drugs.
Absorption
Minimal systemic absorption after topical ocular administration. High local concentrations achieved in ocular tissues.
Metabolism
Primarily local metabolism in ocular tissues; minimal systemic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Moxifloxacin, other quinolones, Ketorolac, other NSAIDs, or any component of the formulation
- Viral or fungal eye infections
- Use in neonates (due to general ophthalmic NSAID precautions)
Drug Interactions
Other ophthalmic medications
Administer at least 5 minutes apart.
Other topical ophthalmic NSAIDs
May increase risk of corneal adverse events.
Topical ophthalmic corticosteroids
May increase potential for healing problems.
Storage
Store at room temperature (below 30°C), away from light and moisture. Do not freeze. Keep out of reach of children. Discard any remaining solution 4 weeks after opening.
Overdose
Accidental ocular overdose is unlikely to cause systemic toxicity due to minimal absorption. If swallowed, supportive care is recommended. Local irritation may occur with excessive topical application.
Pregnancy & Lactation
Use during pregnancy and lactation only if the potential benefit outweighs the potential risk to the fetus or infant. Consult a physician. Animal studies with systemic Moxifloxacin showed developmental toxicity at high doses. Systemic NSAIDs are generally avoided in late pregnancy.

Lab Monitoring
- No specific laboratory monitoring required for routine ophthalmic use.
Doctor Notes
- Caution in patients with a history of corneal epithelial breakdown or healing delay.
- Monitor for signs of increased intraocular pressure with prolonged use (though less likely than corticosteroids).
- Advise patients not to wear contact lenses during treatment.
Patient Guidelines
- Do not wear contact lenses during treatment for bacterial conjunctivitis or post-operative period.
- Avoid contaminating the dropper tip.
- Complete the full course of treatment even if symptoms improve.
- Report any severe or persistent eye irritation or vision changes to your doctor.
Missed Dose Advice
If a dose is missed, apply it as soon as remembered. If it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
May cause temporary blurred vision or discomfort after instillation. Patients should exercise caution when driving or operating machinery until vision clears.
Lifestyle Advice
- Maintain good hand hygiene.
- Avoid rubbing eyes.
