Description
Overview of the medicine
Formet V is a combination medication for the management of type 2 diabetes mellitus. It contains Vildagliptin, a Dipeptidyl Peptidase-4 (DPP-4) inhibitor, and Metformin, a biguanide. This combination helps to improve glycemic control in adult patients.
Uses & Indications
Dosage
Adults
Recommended starting dose is 50 mg Vildagliptin / 500 mg Metformin once or twice daily, taken with meals. The dose can be titrated based on efficacy and tolerability, up to a maximum of 100 mg Vildagliptin and 2000 mg Metformin per day.
Elderly
Dosage should be individualized based on renal function. Close monitoring of renal function is necessary.
Renal_impairment
Not recommended for patients with eGFR <30 mL/min/1.73m². Dose adjustments needed for eGFR 30-59 mL/min/1.73m².
How to Take
Take orally with meals to reduce gastrointestinal side effects associated with Metformin.
Mechanism of Action
Vildagliptin inhibits the DPP-4 enzyme, which leads to increased levels of active incretin hormones (GLP-1 and GIP). These hormones enhance glucose-dependent insulin secretion and suppress glucagon secretion. Metformin reduces hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
Pharmacokinetics
Onset
Vildagliptin: plasma concentrations peak within 1-2 hours. Metformin: peak plasma concentrations within 2-3 hours for immediate release.
Excretion
Vildagliptin: 85% renal excretion. Metformin: renal excretion (primarily tubular secretion).
Half life
Vildagliptin: approximately 3 hours. Metformin: plasma elimination half-life is approximately 4-9 hours.
Absorption
Vildagliptin: rapidly absorbed, absolute oral bioavailability of 85%. Metformin: absolute bioavailability of 50-60%, absorption is incomplete and saturable.
Metabolism
Vildagliptin: primarily hydrolyzed (85%) with a minor role for CYP450 enzymes. Metformin: excreted unchanged in the urine.
Side Effects
Contraindications
- •Hypersensitivity to Vildagliptin or Metformin or any excipients.
- •Renal impairment (eGFR <30 mL/min/1.73m²).
- •Acute or chronic metabolic acidosis, including diabetic ketoacidosis.
- •Acute heart failure or severe cardiac or respiratory insufficiency.
- •Severe hepatic impairment.
- •Acute myocardial infarction.
- •Conditions associated with hypoxemia or dehydration.
- •Acute alcohol intoxication or alcoholism.
- •Concurrent administration of iodinated contrast agents (temporarily discontinue).
Drug Interactions
Alcohol
Increases the risk of lactic acidosis, especially in fasting, malnourished, or hepatically impaired patients.
Iodinated contrast agents
Discontinue Formet V before or at the time of the procedure and restart 48 hours later if renal function is stable, due to increased risk of lactic acidosis.
Glucocorticoids, sympathomimetics, diuretics, thyroid products
May increase blood glucose, requiring dose adjustment of antidiabetic agents.
Cationic drugs (e.g., cimetidine, amiloride, trimethoprim, ranitidine, vancomycin)
May increase metformin accumulation and risk of lactic acidosis by competing for renal tubular transport.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of Formet V, particularly due to Metformin, can lead to lactic acidosis, which is a medical emergency. Symptoms include malaise, myalgia, respiratory distress, abdominal pain, and hypothermia. If an overdose occurs, discontinue Formet V and provide supportive treatment. Hemodialysis is the most effective way to remove Metformin and lactate from the body.
Pregnancy & Lactation
Formet V should not be used during pregnancy unless clearly necessary, as adequate studies in pregnant women are lacking. Metformin crosses the placenta. Vildagliptin and/or Metformin are excreted in breast milk; therefore, it is advised to avoid Formet V during lactation.
